Multidimensional Evaluation of AI-Generated Patient Information on Cone Beam–Computed Tomography

Öçbe, Melisa; Akçay, Hülya Çerçi; Sır, Eda; Taş, Çağan; Erbalta, Rana

This study aimed to conduct a multidimensional evaluation of artificial intelligence (AI) chatbot–generated patient information regarding cone beam–computed tomography (CBCT) in dentistry, with specific focus on readability, informational quality, reliability, and patient-centered suitability. Twenty frequently asked, patient-oriented questions related to CBCT were systematically identified from a public online forum. Each question was submitted to four large language model–based chatbots (ChatGPT-4o, Gemini Advanced, Claude Sonnet 4, and Microsoft Copilot) under standardized conditions. Generated responses were evaluated using validated instruments, including the DISCERN tool and Global Quality Scale (GQS) for information quality and reliability, as well as Flesch Reading Ease, Flesch–Kincaid Grade Level, and Gunning Fog Index for readability. Patient-centeredness was further assessed using PEMAT-Understandability and PEMAT-Actionability scores. Comparative analyses were performed using linear mixed-effects models. Significant differences were observed among chatbots across all evaluated domains (p < 0.05). While advanced models demonstrated higher informational quality and reliability, their responses frequently exceeded recommended health literacy thresholds. Readability, transparency, and actionability varied substantially between platforms. No chatbot consistently met all criteria for optimal patient-directed communication. AI chatbots can provide generally accurate information on CBCT; however, variability in readability, reliability, and educational suitability limits their standalone use for patient education. Careful integration with professional oversight is essential to ensure safe and accessible AI-supported communication in dentomaxillofacial radiology. This study provides the first multidimensional, comparative evaluation of leading AI chatbots in delivering patient-oriented information about cone beam–computed tomography. It shows critical gaps between informational accuracy and health literacy suitability. This study reveals the need for professional oversight when using AI for patient education in dentomaxillofacial radiology.

Evaluating YouTube videos on oral potentially malignant disorders and oral cancer

Akçay, Hülya Çerçi; Kargu, Erdal Cem; Öçbe, Melisa

Background Oral potentially malignant disorders (OPMDs) are abnormalities with an elevated risk of transforming into oral squamous cell carcinoma (OSCC). Despite the visibility of early lesions, OSCC is frequently diagnosed at advanced stages due to low public awareness. YouTube offers potential for oral health education, but the reliability of OPMD-related content remains uncertain. Methods This study systematically assessed the content, reliability, and educational value of 100 English-language YouTube videos on OPMDs and oral cancer. Two independent evaluators analyzed videos using a 6-domain content rubric, JAMA benchmarks, DISCERN, and the Global Quality Scale (GQS). Inter-observer agreement was calculated. Results Of the 100 videos screened, 36 met inclusion criteria. The mean view count was 6,741.9 ± 14,315.5, with limited engagement (interaction index = 1.9 ± 2.3). The highest content scores were observed for Definition and Classification (4.47 ± 0.63) and Diagnostic Approaches (4.07 ± 1.05), while Patient Education ranked lowest (2.57 ± 1.09). Overall video quality was moderate (GQS = 2.94 ± 0.75; DISCERN = 47.29 ± 15.55; JAMA = 2.16 ± 0.67; VIQI = 14.13 ± 2.13). Strong correlations existed among quality indices (ρ = 0.70–0.75, P < .001), but engagement did not correlate with quality. Conclusions OPMD-related YouTube content frequently lacks comprehensive patient education, and credible sources are not always the most viewed. Developing accurate, accessible, and engaging educational videos remains essential.

Cognitive-level analysis of dentomaxillofacial radiology questions in the Turkish dentistry specialization examination: a Bloom’s revised taxonomy analysis

Ocbe, Melisa; Akcay, Hulya Cerci; Gumru, Birsay; Yalcinkaya, Sebnem Ercalik

Background: Bloom’s revised taxonomy is widely used to evaluate the cognitive level of examination questions. With the growing use of large language models (LLMs), interest has increased in their potential to support cognitive classification. However, evidence comparing LLM-based classification with expert consensus in dental specialty examinations remains limited. This study aimed to analyze the cognitive levels of dentomaxillofacial radiology (DMFR) questions in the Turkish Dentistry Specialization Examination (DUS) and to evaluate the agreement between expert consensus and ChatGPT v5.2. Methods: A total of 130 text-based DMFR questions from the DUS were classified according to Bloom’s revised taxonomy using expert consensus and ChatGPT v5.2. Agreement between classifications was assessed using exact agreement and quadratic-weighted Cohen’s kappa. Results: “Analyze” was the most frequent cognitive level in both expert consensus (41.5%) and ChatGPT classification (44.6%), followed by “Remember.” Exact agreement between ChatGPT and expert consensus was 80.0%, with almost perfect ordinal agreement (κw = 0.851; 95% bootstrap CI: 0.769–0.923). Conclusions: ChatGPT v5.2 showed high agreement with expert consensus for Bloom-level classification in this dataset. These findings suggest that LLM-based classification may support Bloom-level mapping as an assistive tool; however, they should be interpreted as preliminary evidence of feasibility rather than practical validation. Expert oversight remains necessary, and further research is needed to assess performance across different models, settings, and educational contexts.

Assessing the reliability and educational value of YouTube videos on computer-controlled local anesthesia in dentistry

Akcay, Hulya Cerci; Kargu, Erdal Cem; Seker, Nefise

Objective This study aimed to assess the scientific accuracy, content quality, and educational value of YouTube™ videos related to computer-controlled local anesthesia (CCLA) techniques in dentistry. Materials and methods A total of 100 videos were screened using predefined keywords, and 48 met the inclusion criteria. Videos were assessed using the Global Quality Scale (GQS), DISCERN tool, JAMA benchmark criteria, and the Video Information and Quality Index (VIQI). Scientific content was scored using a structured rubric across six domains. Interobserver reliability was evaluated using Weighted Kappa and Intraclass Correlation Coefficient (ICC). Confidence intervals were calculated for key metrics. Group comparisons were performed using the Mann-Whitney U test, and correlations were analyzed using Spearman’s rho (p < 0.05). Results Videos from academic sources had significantly higher scores across all quality and reliability indicators. The mean GQS was 2.6 (95% CI: 2.3–2.9), DISCERN 11.7 (95% CI: 10.8–12.6), JAMA 1.8 (95% CI: 1.7–1.9), and VIQI 12.5 (95% CI: 11.7–13.3). Strong positive correlations were found between DISCERN and VIQI (r = 0.809), and between total content score and both DISCERN (r = 0.803) and VIQI (r = 0.655). Conclusion Although YouTube™ provides accessible information on CCLA, many videos lack scientific rigor and educational depth. Content produced by academic institutions is significantly more reliable. Dental educators are encouraged to integrate high-quality video content into curricula to improve media literacy and student learning outcomes.

Cerci Akcay, Hulya; Safci, Duygu; Kasimoglu, Yelda; Aren, Gamze; Guloğlu, Recep; Coskun, Murat; Tanir, Yaşar; Oren, Meryem Merve; Erdem, Arzu Pinar

Background: The Covid-19 pandemic has disrupted daily routines and lifestyle habits, affecting the psychological well-being and dietary habits of individuals, especially children with autism spectrum disorder (ASD). This study aims to evaluate the susceptibility of children with ASD to changes in nutritional habits, lifestyle, and dental trauma during the pandemic related with oral health and compare them with healthy children. Methods: This study, conducted at Istanbul University, involves children aged 3–14 diagnosed with ASD at the Department of Child and Adolescent Psychiatry, and healthy children who visited the Department of Pedodontics. Parents provide written informed consent, and a questionnaire covering various aspects of their child’s health and habits is used. Intraoral examinations are conducted for both groups, recording any trauma history or signs, and evaluating possible causes and risk factors. All data were obtained and analysed by SPSS software program version 23.0 with descriptive statistics calculation, Chi square test, Kolmogorov Smirnov, the Mann-Whitney U test, t test and McNemar test was used with p value fixed at 0.05. Results: The results show significant differences between children with ASD and the control group in terms of dental health. Children with ASD had lower dmft and DMFT indices, indicating better dental health (p = 0.005). However, the frequency of dental trauma was similar between the groups (p = 0.685). Additionally, the control group exhibited more significant changes in dietary habits during the pandemic, with a higher tendency for snacking throughout the day (p = 0.027). During lockdowns, parents of children with ASD were statistically more likely to provide junk food when their child was irritable (p < 0.001). Changes in physical activity levels also differed, with a more pronounced decrease in the control group (p < 0.001). Furthermore, children with ASD experienced a statistically significant higher rate of disruption in daily routines and inability to participate in social activities and courses (p < 0,001). Conclusion: This study sheds light on the unique challenges faced by children with ASD, particularly in terms of dental health and related behaviors. It underscores the importance of interventions to ensure optimal oral health outcomes for this population, especially during periods of disruption like the Covid-19 pandemic.

The relationship between clinical outcomes of untreated dental caries with dental anxiety, health perceptions and Child-OIDP in primary school-aged children

Akcay, Hulya Cerci; Sahin, Merve; Peker, Kadriye; Erdem, Arzu Pınar

Introduction: This study aimed to investigate the relationship between the clinical consequences of untreated dental caries and children’s sociodemographic characteristics, behavioral factors, perception of general/oral health, dental anxiety, and oral health-related quality of life (OHRQoL) and to identify the predictors of childrens’ OHRQoL. Methods: This cross-sectional study was conducted in a convenience sample consisted of 200 children aged 8–10 years attending a pediatric dentistry clinic. Data were collected through clinical examinations and self-reported questionnaires. Caries severity was assessed using the dmf(t)/DMF(T) and pufa(t)/PUFA(T) indices. The questionnaire encompassed socio-demographic characteristics, the Modified Child Dental Anxiety Scale, oral health behaviours, self rated oral/general health, treatment need and satistaction with teeth, the Child-Oral Impacts on Daily Performances index. Descriptive, bivariate, and multivariate statistics were performed for data analyses. Results: The sample consisted of 98 girls and 102 boys (mean age 9.1 ± 0.81 years). D component of DMFT was correlated with the dental anxiety (p < 0.05), but the PUFA/pufa and DMFT/dmft scores were not correlated. No significant differences were found in the OHRQoL and all domains regarding gender, age, school type and oral health behaviours. The OHRQoL was associated with dental anxiety, self perceived oral/general health, the number of self- reported oral problems, and satisfaction with teeth. Worse oral health self- rating and dissatisfaction with the appearance of teeth were important predictors of worse OHRQoL. Conclusion: Anxiety and children’s subjective evaluation of oral health affect children’s OHRQoL. As a part of needs assesment, dentists should consider these factors together with clinical parameters when planning treatment.
