Investigation of evidence-based practices of nurses in a rural public hospital: a mixed methods study

Ziyai, Nasibe Yagmur; Sener, Senay; Dikmen, Yurdanur

Background: Evidence-based practice was essential for ensuring high-quality patient care, improving safety, and reducing healthcare costs. As the largest group of healthcare providers, nurses played a pivotal role in implementing evidence-based practice. However, they often encountered multiple personal and organizational barriers, particularly in rural settings characterized by limited resources and geographic isolation. Aim: This study aimed to comprehensively examine nurses’ attitudes toward evidence-based nursing in a rural public hospital and to assess how these attitudes influenced clinical care practices. Methods: An explanatory sequential mixed-methods design was employed. Quantitative data were collected from 86 nurses using a Nurse Information Form and the Scale of Attitudes Toward Evidence-Based Nursing. Qualitative data were obtained through three mini–focus group interviews with nine nurses purposefully selected based on variations in their evidence-based practice attitudes and professional characteristics. Descriptive statistics, one-way ANOVA, LSD post hoc tests, and thematic analysis were used to analyze the data. Results: The mean attitude score was 58.09 ± 9.17, with significantly higher scores among nurses with 1–10 years of professional experience. Thematic analysis revealed three overarching themes: facilitators, barriers, and suggestions. Facilitators included peer collaboration, innovation, nursing professionalism, and improved quality of patient care. Barriers comprised patient reactions, professional challenges, organizational limitations, insufficient knowledge, and workplace conflicts. Nurses’ recommendations emphasized enhancing patient and nurse education, fostering a culture of discussion, updating clinical guidelines, and strengthening shared decision-making mechanisms. Conclusion: Although nurses demonstrated generally positive attitudes toward evidence-based practice, they encountered substantial challenges in applying it in clinical settings. Establishing structured support systems, leadership training, and policy-level interventions is essential to foster effective and sustainable implementation of evidence-based practice in rural hospitals.

The use of comfort theory in the intensive care unit

    Koç, Ferhat; Dikmen, Yurdanur

Background: A well-functioning intensive care unit should support the patient's recovery by promoting their participation in treatment and care. Enhancing patient comfort in the intensive care unit can maximise the quality of nursing care and shorten the recovery period. Aim: To evaluate the effects of nursing interventions based on comfort theory on comfort levels and perceived environmental stressors of patients in the intensive care unit. Methods: A randomised controlled trial was conducted between 15 October 2019 and 15 February 2020, including 50 intensive care unit patients divided into intervention (n=25) and control (n=25) groups. The intervention group received structured nursing care plans designed according to comfort theory, while the control group received standard intensive care unit care. Data collection tools included the Patient Identification Form, General Comfort Scale and Intensive Care Unit Environmental Stressor Scale. Data analysis used parametric tests to assess differences between groups. Results: The intervention group had a significantly higher comfort level (General Comfort Scale score: 142±28±10.84) compared to the control group (General Comfort Scale score: 111.36±13.34). Additionally, the Intensive Care Unit Environmental Stressor Scale scores, which indicate perceived stressors, were lower in the intervention group (86.25±14.38) compared to the control group (111.80±14.25), suggesting that structured nursing care significantly reduced environmental stress. Conclusions: Findings demonstrated that nursing interventions structured according to comfort theory improved patient comfort levels and reduced environmental stressors in intensive care unit patients. The study highlights the importance of patient-centered nursing care and technology-assisted interventions in enhancing patient wellbeing in critical care settings. Further research is recommended to explore the long-term benefits of these interventions in diverse intensive care unit populations.

A comparison of 3D game-based simulation versus traditional methods in vital signs education

Tanrıkulu, Fatma; Gündoğdu, Handenur; Erol, Funda; Dikmen, Yurdanur

Background: Vital signs measurement is a fundamental nursing responsibility that requires cognitive and psychomotor competence. In alignment with the ethical principle of nonmaleficence, it is important to provide learning environments where students can practice and make mistakes without causing harm. This study aims to examine the effect of a 3D game-based simulation application developed for vital signs on students’ academic achievement, learning satisfaction and self-confidence levels. Methods: A randomized controlled experimental design was employed with 73 nursing students, assigned to either the intervention group (n = 37) or the control group (n = 36). The intervention group used a game-based simulation developed with 3D animation technology, while the control group received traditional instruction. Data were collected using the Student Introductory Characteristics Form, Academic Achievement Test of Vital Signs Measurement Skills, and the Student Satisfaction and Self-Confidence in Learning Scale. Statistical analyses were conducted using SPSS version 25, with significance set at p < 0.05. Results: The results indicated that students’ current learning satisfaction subscale scores were high in the traditional teaching group and that there was a statistically significant difference between the groups (p = 0.047). There was no statistically significant difference between the groups in terms of academic achievement (p = 0.932) and self-confidence levels in learning (p = 0.375). However, both groups had high mean scores in academic achievement, satisfaction with current learning, and self-confidence in learning. Conclusions: The game simulation method supported by 3D animation may be insufficient for significantly improving nursing students’ academic performance, satisfaction with the learning process, and self-confidence in learning. Therefore, it is recommended that such technology-supported methods be used in blended learning environments alongside traditional teaching methods in nursing education. Trial registration: ClinicalTrials.gov: NCT07009275; retrospectively registered.

Background: Violence against women is a global public health and human rights issue with profound physical and psychological consequences. Simulation-based learning may help nursing students recognize signs of violence. This study examined whether physical violence indicators created on a standardized patient using moulage improve nursing students’ recognition of violence and their related attitudes, knowledge, satisfaction, self-efficacy, and communication skills. Methods: A randomized controlled pretest–posttest design was conducted with 55 fourth-year nursing students allocated to a moulage group (n = 28) and a control group (n = 27). The data collection tools included the “Demographic Information Form”, “ISKEBE Attitude Scale for Violence Against Women,” “Communication Skills Assessment Form”, “Nurses’ and Midwives’ Recognition of Signs of Violence Against Women Form”, “Modified Simulation Effectiveness Tool”, “Student Satisfaction and Self-Efficacy Scale in Learning,” and “Moulage Evaluation Questionnaire.” Data analysis involved calculating frequency, mean, standard deviation, chi-square tests, the Mann-Whitney U test, and the Wilcoxon Signed Ranks Test. Results: The demographic data of the moulage and control groups were similar. Recognition of violence indicators was significantly higher in the moulage group (19.75 ± 2.78) than in the control group (16.38 ± 3.63; p < 0.05). The communication skills of the students in the moulage group had an average score of 5.35 ± 0.48 for the effective communication subscale, while the average score for the therapeutic communication subscale was 4.55 ± 2.91. Additionally, the moulage group reported greater satisfaction with current learning (51.85 ± 6.66) and higher self-efficacy (29.21 ± 4.26, p = 0.006) than the control group. The mean Modified simulation effectiveness tool score was 68.42 ± 20.87, indicating a positive perception of learning effectiveness. No significant between-group difference was found for attitudes (ISKEBE). Conclusion: Moulage-supported simulation enhanced recognition of physical violence indicators, satisfaction, self-efficacy, and communication-related outcomes. The integration of these methods is recommended in future educational programs. Clinical registration: The study is registered on ClinicalTrials.gov. ID: NCT06982017 (date: 2024-05-20).
