Intro: In this prospective interventional study, we aimed to assess the efficacy and safety of the hepatitis A vaccine in patients with autoinflammatory diseases undergoing canakinumab and tocilizumab treatments. Methods: A total of 24 patients with autoinflammatory diseases on canakinumab and tocilizumab treatments and 39 healthy children who were seronegative for hepatitis A were included in the study. All participants were vaccinated with 2 doses of inactivated hepatitis A vaccine at 6-month intervals. One month after the last vaccination, venous blood samples were collected from each participant, and the anti-HAV IgM and IgG titers were measured. Results: The patient group consisted of 19 patients with systemic juvenile idiopathic arthritis (sJIA) and 5 patients with cryopyrin-associated periodic fever. The mean age of the patient group and the healthy controls were 14.1 ± 3.7 and 12.4 ± 3.2 years, respectively. All patients with cryopyrin-associated periodic fever and 52.6% (10/19) of the patients with sJIA were on canakinumab. The remaining 9 patients (47.3%) with sJIA were using tocilizumab. Among sJIA patients, 15 were also on methotrexate, and 14 were on prednisolone. All of the participants were negative for hepatitis A serology. After two doses of hepatitis A vaccine, all the patients with autoinflammatory diseases (24/24) and 84.6% (33/39) of the healthy controls were detected as positive for anti-HAV IgG (p = 0.04). The mean anti-HAV Ig G titers of the patient group and the control group one month after the last dose of vaccination were 5.25 ± 1.49 IU/L and 10.5 ± 7.02 IU/L, respectively (p < 0.001). Neither disease flares nor adverse effects related to vaccination were observed within the study period. Conclusion: Hepatitis A vaccine is effective and safe in children with autoinflammatory diseases on biologic therapy. Long-term follow-up data, including larger patient cohorts, are needed to draw a solid conclusion about the safety of hepatitis A vaccine in patients receiving canakinumab and tocilizumab therapy.

Objective: In our study, it was aimed to analyze the antioxidant and antimicrobial activities of the extracts of the fruits of Rhus coriaria L. (sumac) species collected from Gaziantep. Methods: Ethanol extracts of 80% (R2) and 100% (R3) were prepared from Rhus coriaria fruits. Chemical analysis of the extracts were performed by liquid chromatography-high resolution mass spectrometry method, their antioxidant activities were investigated by 1,1-diphenyl-2-picrylhydrazyl (DPPH) free radical scavenging activity, and their antimicrobial activity was investigated using broth microdilution method. Results: In the chemical analysis of R2 and R3 extracts, fumaric acid, an organic acid with the highest concentration, was found at concentrations of 31076.55 and 23348.37 mg/kg, respectively. While the phenolic components with the highest concentration in R2 were observed as hyperoside (622.24 mg/kg), ellagic acid (343.63 mg/kg) and p-coumaric acid (182.91 mg/kg), the phenolic components with the highest concentration in R3 were observed as ellagic acid (607.30 mg/kg), hyperoside (440.41 mg/kg) and p-coumaric acid (178.61 mg/kg). In antioxidant activities of R2 and R3 extracts, DPPH free radical scavenging activities were found to be 70.78%+/- 0.002% and 11.19 +/- 0.001%, respectively. Antimicrobial activities of R2 and R3 extracts were found to be 125 and <3.9 mu g/ mL in S. aureus strain ATCC 25923, 15.625 and 31.25 mu g/mL in A. baumannii strain ATCC 19606, 62.5 mu g/mL in H. pylori strain ATCC 43504, 62.5 mu g/mL in C. glabrata strain ATCC 2001, <3.9 mu g/mL in C. albicans strain ATCC 66027, respectively. Conclusion: The higher antioxidant activity in the R2 extract obtained from R. coriaria fruits grown in our country may be due to the higher phenolic component content compared to the R3 extract. It is thought that the more effective antimicrobial activity detected in the R3 extract may be due to the higher amount of ellagic acid compared to the R2 extract.

Tikvesli, Melek; Mayda, Pelin Yüksel; Kuloglu, Figen

Infectious diseases such as tuberculosis (TB) are leading causes of human death. Antibiotics are effective molecules to combat bacterial infections by affecting the processes required for bacterial cell growth and proliferation. The development of new antibiotics has become an important issue as overdosed or incorrect use of antibiotic lead to the development of antibiotic resistance. In this study, a new series of 4-(1,2,3-triazoyl)arylmethanone derivatives has been synthesized using the one-pot Copper-Catalyzed Oxidative Cross-Dehydrogenative Coupling/Oxidative Cycloaddition strategy to overcome the aforementioned problems. New compounds were characterized by using spectroscopic techniques (1H, 13C-APT-NMR, FT-IR, and HRMS-TOF). In vitro antimycobacterial activities of the arylmethanone derivatives against the Mycobacterium tuberculosis H37Rv standard strain were examined using the resazurin microplate method in the presence of streptomycin and rifampycin as standard medicines. Bioactive assays demonstrated promising results that some of the synthesized 4-(1,2,3-triazoyl)arylmethanone derivatives exhibited good anti-TB activities. Notably, compounds 6b, 6f, and 6g gave the most potent efficiency with minimum inhibitory concentration values of 4 μg/ml (12.8, 11.7, and 12.8 μΜ, respectively). Among the synthesized compounds, the cytotoxicity measurements of the 6b, 6f, 6g, 6d, and 6v derivatives were screened and it was found that the 6f derivative did not show any cytotoxicity. Molecular docking analyses are utilized to identify the binding mode and the key interactions between target compounds and the ligand-binding site of M. tuberculosis enoyl-acyl carrier protein reductase enzyme (MtInhA, EC 1.3.1.9). The docking results were in agreement with the in vitro results, which confirm the synthesized ligands bind to MtInha with moderate to low affinity.

Background and Objectives: Helicobacter pylori infection is associated with chronic gastritis, ulcers, and gastric cancer. The H. pylori Type 4 secretion system (T4SS) translocates the CagA protein into host cells and plays an essential role in initiating gastric carcinogenesis. The CagL protein is a component of the T4SS. CagL amino acid polymorphisms are correlated with clinical outcomes. We aimed to study the association between CagL amino acid polymorphisms and peptic ulcer disease (PUD) and non-ulcer dyspepsia (NUD). Materials and Methods: A total of 99 patients (PUD, 46; NUD, 53) were enrolled and screened for H. pylori by qPCR from antrum biopsy samples. The amino acid polymorphisms of CagL were analyzed using DNA sequencing, followed by the MAFFT sequence alignment program to match the amino acid sequences. Results: Antrum biopsy samples from 70 out of 99 (70.7%) patients were found to be H. pylori DNA-positive. A positive band for cagL was detected in 42 out of 70 samples (PUD, 23; NUD, 19), and following this, these 42 samples were sequenced. In total, 27 different polymorphisms were determined. We determined three CagL amino acid polymorphism combinations, which were determined to be associated with PUD and NUD. Pattern 1 (K35/N122/V134/T175/R194/E210) was only detected in PUD patient samples and was related to a 1.35-fold risk (p = 0.02). Patterns 2 (V41/I134) and 3 (V41/K122/A171/I174) were found only in NUD patient samples and were linked to a 1.26-fold increased risk (p = 0.03). Conclusions: We observed three new patterns associated with PUD and NUD. Pattern 1 is related to PUD, and the other two patterns (Patterns 2 and 3) are related to NUD. The patterns that we identified include the remote polymorphisms of the CagL protein, which is a new approach. These patterns may help to understand the course of H. pylori infection.
