Percutaneous closure of a giant left ventricular pseudoaneurysm using an Amplatzer muscular VSD occluder in a high-risk patient with recurrent heart failure following multiple myocardial infarctions

Coskun, Senol; Sag, Saim; Ozdabakoglu, Osman; Kilic, Teoman

Background – Left ventricular pseudoaneurysm (LVPA) is a rare and potentially fatal complication of myocardial infarction (MI), with mortality reaching 48% in untreated patients. While surgical repair has been the conventional therapy, perioperative mortality of 10%–36% renders it prohibitive in patients with multiple comorbidities. Percutaneous transcatheter closure has emerged as a life-saving alternative for high-risk surgical candidates. Case report – A 75-year-old male with prior coronary artery bypass grafting (2010) and two subsequent myocardial infarctions (2019 and 2020) treated with percutaneous coronary intervention presented with refractory decompensated heart failure, massive left pleural effusion, and markedly elevated NT-proBNP (7, 172 pg/mL). Cardiac magnetic resonance imaging revealed a giant LVPA (61 mm × 28 mm; end-diastolic volume 55 mL) arising from the lateral-to-posterolateral wall, with transmural late gadolinium enhancement confirming a post-infarction etiology. After multidisciplinary heart-team consensus, percutaneous transcatheter closure was performed via a transfemoral retrograde approach under transesophageal echocardiographic guidance using a 16 mm Amplatzer Muscular VSD Occluder. The procedure was uncomplicated, with no significant residual flow. The patient was discharged on postoperative day 2. At 3-month follow-up, he was in NYHA class I-II with marked symptomatic improvement, NT-proBNP reduction to ∼500 pg/mL, and no residual flow. Conclusions – Percutaneous transcatheter closure with an Amplatzer Muscular VSD Occluder is a feasible, safe, and life-saving therapeutic option in selected high-risk patients with post-infarction LVPA presenting with refractory heart failure.

Attitudes and behaviors toward hypnosis and hypnotic susceptibility in physiotherapy patients: a cross-sectional study

Kuculmez, Ozlem; Dundar Ahi, Emine; Ozbek, Ilhan Celil; Afsar, Sevgi Ikbali

Background and objectives: Hypnosis is a focused cognitive state with increased suggestibility and neurophysiological changes, but misconceptions remain despite its clinical value. This study examined physiotherapy patients' beliefs and behaviors about hypnosis, their hypnotic suggestibility levels, and the relationships among these factors. Materials and methods: This cross-sectional study included voluntary adult participants aged 18–70 years who presented to the physical medicine and rehabilitation outpatient clinics. Individuals with psychiatric disorders, cognitive impairment, poor general health, illiteracy, or incomplete data were excluded. Demographic characteristics were recorded, and participants were asked to complete the attitudes and beliefs toward hypnosis scale (VSABTH-T) and the Short Form of the Suggestibility Scale (SSS). The study evaluated participants' attitudes, beliefs, and levels of suggestibility, as well as the relationships among these variables. A significance level of p < 0.05 was accepted for all statistical analyses. Results: The study analyzed data from 399 participants (65.9% male, 34.1% female) with a mean age of 42.68 ± 16.75 years. 75.4% of participants had limited knowledge, and 3.5% had hypnosis experience. Predominantly expressed concerns regarding hypnotic procedures were 86.2% fear, 61.9% emerging from hypnosis, 51.8% losing control, and 33.3% of the participants defined hypnosis as dangerous. Participants had a mean attitude and behavior score toward hypnosis of 107.21 ± 15.8 and a mean hypnotizability score of 53.29 ± 8.76. Although age, occupation, and income level showed very weak but statistically significant correlations with the total VSABTH-T scores (p = 0.001), there was no correlation with total SSS scores (p > 0.05). Attitudes toward hypnosis and hypnotic suggestibility are significantly affected by age, education, occupation, income, and particularly the level of knowledge about hypnosis (p < 0.05). Total VSABTH-T and SSS scores were not correlated (p = 0.069), although very weak associations were observed between subdimensions (p < 0.05). Conclusion: Physiotherapy patients demonstrated limited hypnosis knowledge and experience and misconceptions. Demographic factors showed weak associations with attitudes and suggestibility. Findings highlight that personal beliefs and demographics interact complexly in shaping hypnotic responsiveness, emphasizing the need for targeted education to reduce misconceptions.

Comparison of safety and short-term outcomes of septostomy and direct tunnel passage techniques in percutaneous patent foramen ovale closure: a multicenter experience

Coskun, Senol; Tiryakioğlu, Selma Kenar; Mirzamidinov, Didar; Inan, Duygu; Kilicgedik, Alev; Sahin, Tayfun; Kilic, Teoman

Background – Percutaneous closure of a patent foramen ovale (PFO) is an established therapeutic strategy for patients with cryptogenic stroke (CS). This study aimed to compare the procedural safety, efficacy, and short-term clinical outcomes of atrial septostomy vs. standard direct tunnel passage techniques for PFO closure. Methods – We retrospectively analyzed consecutive patients who underwent PFO closure for CS at four centers between January 2010 and December 2020. Demographic characteristics, vascular risk factors, Risk of Paradoxical Embolism (RoPE) scores, PFO anatomical features, procedural details, and clinical outcomes were compared between patients treated with atrial septostomy and those undergoing direct tunnel passage. Continuous variables are presented as mean ± standard deviation or median [interquartile range], and categorical variables as counts and percentages. Results – A total of 246 patients were included (mean age 40.5 ± 9.8 years), of whom 69.9% presented with ischemic stroke and 30.1% with transient ischemic attack (TIA). Atrial septostomy was performed in 106 patients (43.1%), while 140 patients (56.9%) underwent direct tunnel passage. Baseline clinical characteristics were comparable between groups; however, the septostomy group exhibited significantly more complex PFO anatomy, including atrial septal aneurysm (69.8% vs. 39.3%), septal hypermobility (62.3% vs. 34.3%), and spontaneous shunting at rest (85.8% vs. 50.7%; all p < 0.001). Procedure duration was similar between groups (41.1 ± 10.2 vs. 40.7 ± 13.8 min; p = 0.83), and technical success was achieved in nearly all cases. No major procedural complications occurred. Minor adverse events were infrequent and comparable (8.4% vs. 4.3%; p = 0.20). At six-month follow-up, residual shunt rates were numerically lower in the septostomy group (6.6% vs. 11.4%; p = 0.27). No TIAs were observed in the septostomy group, whereas a 3.6% incidence was noted in the direct passage group (p = 0.08). New-onset atrial arrhythmia occurred in two patients (1.9%) in the septostomy group and in none of the direct passage group (p = 0.18). Conclusion – Despite being applied in patients with more complex PFO anatomy, atrial septostomy demonstrated comparable procedural safety, technical success, and short-term efficacy to standard direct tunnel passage. Tunnel length emerged as an independent predictor of residual shunt, regardless of closure technique. In experienced centers, atrial septostomy appears to be a safe and practical alternative for anatomically challenging PFO closures.

Alternative diagnoses in neonates undergoing therapeutic hypothermia for presumed hypoxic–ischemic encephalopathy

Donmez, Tugrul; Serce Pehlevan, Ozge; Gunlemez, Ayla

Background – Early identification of hypoxic–ischemic encephalopathy (HIE) remains challenging during the first hours of life, and therapeutic hypothermia (TH) is often initiated before the underlying diagnosis can be established. Objective – The primary aim of this work is to compare the clinical characteristics, laboratory findings, and short-term outcomes of neonates treated with TH for presumed HIE according to their predominant underlying diagnosis. Methods – We retrospectively reviewed all neonates who received TH for suspected HIE between 2019 and 2023. Infants were categorized according to the predominant underlying diagnosis identified following comprehensive clinical, neurophysiological, neuroimaging, metabolic, genetic, and subspecialty evaluations. Demographic, clinical, neurologic, and laboratory parameters—including inflammation- and oxidative stress-related indices—were compared between groups. Results – Among 92 neonates treated with TH, nine (10%) were subsequently found to have non-hypoxic–ischemic etiologies of neonatal encephalopathy. Baseline neurological severity assessed by Sarnat staging and initial amplitude-integrated electroencephalography patterns did not differ between groups. However, infants in the non-HIE group demonstrated significantly higher rates of myocardial injury (77% vs. 22%, p < 0.001) and severe thrombocytopenia (66% vs. 25%, p = 0.016). These infants also had substantially longer hospitalizations [median (25P–75P): 28 (17–38) vs. 7 (5–11) days, p = 0.002] and higher mortality rates (55.6% vs. 6%, p = 0.001). The median day of death was earlier in the non-HIE group [median (25P–75P): 3 (1–57) vs. 16 (8–77) days; p = 0.30]. However, it was not statistically significant. Inflammatory and oxidative stress indices did not differ significantly between groups. Conclusion – Approximately 10% of neonates undergoing therapeutic hypothermia for presumed HIE were ultimately found to have non-hypoxic–ischemic causes of neonatal encephalopathy despite similar early neurologic presentations. These infants experienced greater systemic morbidity and poorer short-term outcomes. While the small and heterogeneous non-HIE cohort warrants cautious interpretation, disproportionately severe systemic manifestations—particularly myocardial injury and thrombocytopenia—may represent early clinical clues prompting broader diagnostic evaluation beyond hypoxic–ischemic injury.

High-frequency laser therapy: a new alternative to physiotherapy in the treatment of cervical disk hernia

Kuculmez, Ozlem; Dündar Ahi, Emine; Cosar, Sacide Nur; Guzel, Sukran

Introduction: High-frequency laser therapy has been increasingly used in several musculoskeletal disorders, but there is still a lack of evidence for the usage of the device in neck pain. This study aimed to compare the effectiveness of physiotherapy, high-frequency laser, and exercise therapy methods in the treatment of pain in cervical disk herniation. Methods: It was a multicenter, randomized, controlled clinical trial. Patients aged between 18 and 65 with neck pain and a diagnosis of cervical disk hernia were included in the study. Patients with a history of cervical surgery, rheumatism, cancer, or pacemaker were excluded from the study. The patients were randomized into 15 sessions of physiotherapy, high-frequency laser, or exercise therapy groups and evaluated with a range of motion, visual analog score, Neck Disability Index, and Short Form Health Survey-36 before treatment, after treatment, and in 1st and 3rd months. p < 0.05 was considered statistically significant. Results: In total, 150 patients were analyzed. There was a significant improvement in range of motion, visual analog score, Neck Disability Index, and Short Form Health Survey-36 scores in three groups after 3-month follow-up (p < 0.05). The improvement was statistically greater in the physiotherapy and high-frequency laser therapy groups (p < 0.05), but there was no significant difference between these two groups (p > 0.05). Discussion: The results in the physiotherapy and high-frequency laser therapy groups were better than the exercise group. They may be alternatives to each other in cervical disk hernia treatment.

Assessment the impact of operator experience on cyclic fatigue resistance in reciprocating and rotary NiTi files: a comparative study between dental students and pediatric dentistry specialists

Özyürek, Hande; Elbay, Mesut; Özyürek, Taha

Introduction: Devices reducing clinical practice time, and which get minimally affected by user experience are invaluable in pediatric dentistry. Additionally, knowing the fracture resistance of different nickel-titanium (NiTi) files for root canal preparation of primary teeth is important for treatment success. Thus, the present study aimed to compare the instrumentation time (IT) and fracture resistance of reciprocating T-endo MUST (TEM) and continuously rotating AF Baby File (ABF) NiTi files according to the use of pediatric dentistry specialists. Methods: According to their clinical experience level, 3 experimental groups (EG)s were formed namely: no experience, 2-years (less-experienced), and 10-years experienced pediatric dentistry specialists' groups. The fourth group was the control group (CG). In each EGs, 10 TEM (25/.06) and 10 ABF 20/.04, and 10 ABF 25/.04 NiTi file systems were used in 20 artificial resin teeth. Instrumentation time was recorded for each tooth. In the CG, the same number of files were not used for instrumentation. The files in all groups were subjected to a dynamic cyclic fatigue test, and the data were analyzed using the Kruskal–Wallis test at 5% significance level. Results: In all groups, TEM files showed a significant higher fatigue resistance as compared to ABF files. However, fatigue resistances were significantly lower in (EG)s both in TEM and ABF compared to (CG)s. The instrumentation time was found to be significantly lower in TEM compared to the ABF. Also, for both file systems, the experience level did not affect the fatigue resistance, and instrumentation time. Discussion: The experience level of the pediatric dentist did not affect the cyclic fatigue resistance of the reciprocating and rotating NiTi files, but the instrumentation time was lower for the reciprocating NiTi files.
