Volumetric and Asymmetric Index Analysis of Subcortical Structures in Multiple Sclerosis Patients: A Retrospective Study Using volBrain Software

Tekin, Ayla; Rende, Buket; Efendi, Husnu; Bunul, Sena Destan; Çakir, Ozgur; Çolak, Tuncay; Balci, Sibel

Introduction Multiple sclerosis (MS) is a chronic and autoimmune disease that has a significant influence on the central nervous system, such as the brain and spinal cord, affecting millions of individuals globally. Understanding the connection between subcortical brain regions and MS is crucial for effective diagnostic and therapeutic approaches for treating this disabling disease. This study explores the relationship between volume and contours of asymmetry index of subcortical brain regions in individuals with MS using volBrain software (https://www.volbrain.net; developed by Jose V. Manjon (Valencia Polytechnic University, Valencia, Spain) and Pierrick Coupe (University of Bordeaux, Bordeaux, France)). Methods In our retrospective investigation, we admitted 100 Turkish individuals, comprising 50 patients diagnosed with relapsing -remitting MS (RRMS) (24 (48%) males and 26 (52%) females) and 50 healthy controls (23 (46%) males and 27 (54%) females), registered between October 2017 and February 2022 for five years and underwent assessment in the radiology department at the Teaching and Research Hospital of Kocaeli University; 1,150 Turkish patients were excluded from our study based on our exclusion criteria. We used magnetic resonance imaging with a 3-Tesla (3T) scanner and volBrain software to assess volumes (cm(3)) and asymmetry indexes due to asymmetry for different levels of atrophy of total intracranial, total brain, gray matter, white matter, and subcortical regions, the most affected regions in MS patients for both patient and control cohorts. Results Statistical analysis revealed a significant difference between patient and control groups (p < 0.001), with patient group mean age at 38.32 years and control group mean age at 32.88 years. Patient group exhibited lower values for total intracranial, total brain, gray matter, white matter, and cerebrospinal fluid volume compared to control group (p < 0.05). The results indicated a statistically significant decrease (p < 0.05) in the values for total intracranial and total brain volume, whereas all other values remained unchanged. We compared volumes of subcortical structures on the right and left sides and found that the putamen, thalamus, and globus pallidus had statistically lower values in the patient group than in the control group (p < 0.001), apart from the lateral ventricle. Furthermore, our retrospective investigation demonstrated a statistically significant difference in the globus pallidus asymmetry index, indicating a preference for the patient group (p < 0.05). A lower asymmetry index value signifies a larger volume for the right side of the subcortical regions of the brain when compared to the left side. Conclusion Brain atrophy, although characterized by irreversible tissue damage, is targeted by therapeutic interventions to prevent progression. It is, therefore, imperative to develop a universally accepted measurement standard for subcortical structures that also considers the inherent variability present within each structure. Our findings serve as an important basis and indicator for the determination of subcortical atrophy and asymmetry in MS, the prognosis of the disease, and the etiology of clinical symptoms. Subsequent research may benefit by adopting the novel approach of considering brain atrophy as an outcome rather than a predictor, thereby facilitating the elucidation of the intricate biological mechanisms that give rise to volume loss.

    Tekin, Ayla; Buket, Rende

Objective: Aim of our study is to get students' opinions about anatomy education, to determine their attitudes towards course, and to investigate these attitudes’ effects on exam anxiety. Methods: Descriptive study population was conducted with 648 student participants studying at Faculty of Dentistry and Health Sciences and Vocational School in Kocaeli Health and Technology University, 2022-2023 academic year. 63 multiple-choice questions were asked to students within scope of 3 different evaluation elements, namely Personal Data Form, Attitude Scale Regarding Anatomy Course and Exam Anxiety Scale. It was aimed to determine students’ opinions and attitudes about anatomy course and their effect on exam anxiety. Results: In our study, it was observed that there was a significant difference between Attitude Scale Towards Anatomy Course and age, class and preferring to department willingly variables (p<0.05). No significant difference was found between gender and education level, and levels of attitude towards anatomy course and exam anxiety (p>0.05). There was a significant difference between anxiety levels in the Exam Anxiety Scale and preferring to class and department willingly variable (p<0.05). Conclusion: Our study shows that the factors affecting students' attitudes towards the anatomy course and exam anxiety levels are variables such as age range, class level and the status of coming to the department willingly. In this context, in order to develop a positive attitude towards the anatomy course and to reduce exam anxiety, it is recommended to organize the course content in accordance with the learning levels, to strengthen the teacher-student interaction and to use interactive learning methods. The results of this research may contribute to the effective design and implementation of the information given in anatomy education by supporting healthcare professionals to have higher academic success and to continue their professional practice in a more qualified manner.

Hip Joint Angular Values in Children with Bilateral Spastic Cerebral Palsy: A Comparison between Ambulatory and Nonambulatory Groups According to the Gross Motor Functional Classification System

    Rende, Buket; Orha, Ayla Tekin

Objective: The aim of this study was to compare the migration index (MI), femoral inclination angle (FIA), lateral center-edge angle (LCEA), and acetabular index (AI) values measured from hip joint radiographs of children with bilateral spastic cerebral palsy (CP) divided into ambulatory and nonambulatory groups according to the Gross Motor Functional Classification System with functional levels of children, and to examine their relationship with anthropometric measurements of the lower extremities. Materials and Methods: The present investigation encompassed the evaluation of 30 children afflicted with CP, through the conduction of hip joint radiographic assessments, which involved the right and left hip joints, the acquisition of images using MI, FIA, LCEA, and AI parameters, as well as measurement of lower-extremity anthropometric data. Results: A significant difference was found between ambulatory and nonambulatory groups in the left MI, FIA, and LCEA (P < 0.05). In comparison of anthropometric measurements to height ratios, no significant difference was found, except for left ankle circumference and bi-iliac diameter measurements (P > 0.05). Conclusion: The hip joint angular values of the ambulatory group were found to be within normal limits compared to those of the nonambulatory group. Although there were significant differences between hip joint angles in the two groups, there was no significant correlation with lower-extremity anthropometric measurements. It was concluded that hip joint angles were more pathological and prone to hip displacement in the nonambulatory group. This study revealed the importance of strategies to maintain and improve the functional capacity of children with CP.
