Evaluation of Patients’ Preferences for Dentists’ Clinic Attire: A Cross-Sectional Study

    Akbulut, Yasin; Akgün, Semih Ercan

Purpose: To evaluate the preferences of pediatric, adolescent, and young adult patients regarding dentists’ clothing. Materials and Methods: Five hundred and forty patients admitted to the Orthodontics and Pediatric Dentistry Departments of XXX University Faculty of Dentistry who wanted to participate in the study were randomly selected. The patients were grouped as children (7-11 years; n=180; group 1), adolescents (12-17 years; n=180; group 2), and young adults (18-25 years; n=180; group 3). Patients were shown a template consisting of eight different types of clothing and their preferences were recorded. Uniforms with different colors between uniform no. 3-8 were classified as a single preference under the name of ‘colored uniform’ to determine general preferences and were evaluated separately under the title of general preferences. Results: The white coat was the most preferred clothing among eight preferences by the participants (23.5%). Uniform number 3, uniform number 5, and white coat were the most preferred clothing in Group 1 (26.1%), Group 2 (22.8%), and Group 3 (55%), respectively. The general preference among the 3 groups was statistically significant (p < 0.001). In general preferences, colored uniforms were preferred the most (68.1%), and civilian clothing was preferred the least (8.3%). Colored uniforms were preferred mostly in Groups 1 and 2 (92.2% and 79.4%, respectively), and a white coat was the most common preference in Group 3 (55%). Conclusions: Dentists’ wearing the attire most preferred by patients of different ages can significantly increase patient satisfaction and patients' perceptions of the care they receive.

Effects of Rapid Maxillary Expansion on Pulmonary Function in Adolescents: A Spirometric Evaluation

Akbulut, Yasin; Oksayan, Rıdvan; Sokucu, Oral; Isman, Nurettin Eren; Demir, Tuncer

Objectives: Rapid maxillary expansion (RME) is widely used in orthodontics to correct transverse maxillary deficiencies. Beyond its skeletal and dental effects, RME may influence upper airway dimensions and respiratory function, particularly in growing individuals. This study aimed to evaluate the impact of RME on pulmonary function in adolescents using spirometric measurements. Materials and Methods: Fifteen adolescent patients (8 females, 7 males; mean age: 13.93 ± 2.89 years) diagnosed with maxillary transverse constriction underwent orthodontic treatment with acrylic-bonded RME appliances over a mean duration of 3.56 ± 0.67 months. Respiratory function was assessed via spirometry at baseline (T0) and one day after appliance removal (T1). Parameters recorded included peripheral oxygen saturation (SpO2), forced expiratory volume in one second (FEV1), forced vital capacity (FVC), FEV1/FVC ratio, and vital capacity (VC). Data were analyzed using the paired-samples t-test (for normally distributed variables) or the Wilcoxon signed-rank test (for non-normal distributions), with statistical significance set at p < 0.05. Results: Following RME treatment, all respiratory parameters showed a consistent upward trend but did not reach statistical significance. SpO2 increased from 96.98 ± 0.96% to 97.01 ± 0.98% (p = 0.925). VC rose from 2.86 ± 1.07 L to 3.03 ± 0.80 L (p = 0.626). The FEV1/FVC ratio improved from 90.88 ± 12.17% to 92.34 ± 7.37% (p = 0.742). Mean FEV1 increased from 2.61 ± 0.72 L to 2.72 ± 0.68 L (p = 0.518), while FVC rose from 2.87 ± 0.75 L to 2.96 ± 0.69 L (p = 0.547). No adverse effects were reported during the treatment period. Conclusions: This study identified a non-significant but consistent trend toward improved pulmonary function following RME in adolescents. These preliminary findings should be considered hypothesis-generating rather than confirmatory evidence, as none of the outcomes reached statistical significance. While the observed upward trends in oxygen saturation, lung volumes, and expiratory performance suggest potential respiratory benefits, larger-scale, controlled, and long-term studies incorporating both spirometric and anatomical airway assessments are needed to validate these observations.

Optimizing Miniscrew Stability: A Finite Element Study of Titanium Screw Insertion Angles

    Akbulut, Yasin; Ozdemir, Serhat

This study aimed to evaluate how different insertion angles of titanium orthodontic miniscrews (30°, 45°, and 90°) influence stress distribution and displacement in surrounding alveolar bone using three-dimensional finite element analysis (FEA), with a focus on biomechanical outcomes at the titanium–bone interface. The 90° insertion angle generated the highest stress in cortical bone (58.2 MPa) but the lowest displacement (0.023 mm), while the 30° angle produced lower stress (36.4 MPa) but greater displacement (0.052 mm). The 45° angle represented a compromise, combining moderate stress (42.7 MPa) and displacement (0.035 mm). This simulation-based study was conducted between January and April 2025 at the Department of Orthodontics, Kocaeli Health and Technology University. A standardized 3D mandibular bone model (2 mm cortical and 13 mm cancellous layers) was constructed, and Ti-6Al-4V miniscrews (1.6 mm × 8 mm) were virtually inserted at 30°, 45°, and 90°. A horizontal orthodontic load of 2 N was applied, and von Mises stress and displacement values were calculated in ANSYS Workbench. Stress patterns were visualized using color-coded maps. The 90° insertion angle generated the highest von Mises stress in cortical bone (50.6 MPa), with a total maximum stress of 58.2 MPa, followed by 45° (42.7 MPa) and 30° (36.4 MPa) insertions (p < 0.001). Stress was predominantly concentrated at the cortical entry point, especially in the 90° model. In terms of displacement, the 90° group exhibited the lowest mean displacement (0.023 ± 0.002 mm), followed by 45° (0.035 ± 0.003 mm) and 30° (0.052 ± 0.004 mm), with statistically significant differences among all groups (p < 0.001). The 45° angle showed a balanced biomechanical profile, combining moderate stress and displacement values, as confirmed by post hoc analysis. From a biomimetics perspective, understanding how insertion angle affects bone response provides insights for designing bio-inspired anchorage systems. By simulating natural stress dissipation, this study demonstrates that insertion angle strongly modulates miniscrew performance. Vertical placement (90°) ensures rigidity but concentrates cortical stress, whereas oblique placement, particularly at 45°, offers a balanced compromise with adequate stability and reduced stress. These results emphasize that beyond material properties, surgical parameters such as insertion angle are critical for clinical success.
