Parents' Needs and Their Perceived Obstacles With Team Members in Perinatal Palliative Care: A Qualitative Phenomenological Study

    Yasar, Beril Nisa; Terzioglu, Fusun

Advances in prenatal diagnosis have made medical, psychological, and ethical issues regarding the diagnosis of fetal anomalies a crucial topic of research. This study identified the needs of parents of unborn babies with fetal anomalies and their views regarding the services of interdisciplinary team members in charge of assisting them. The sample consisted of 30 parents and 40 team members from a women's health education and research hospital. Data were collected through face-to-face interviews and were qualitatively analyzed using the phenomenological method of interpretation. The results showed that parents' physical, psychological, social, and spiritual needs during pregnancy, childbirth, and postpartum were not fully met after their unborn babies were diagnosed with fetal anomalies. The results also showed that team members needed a structured program, legal regulations, and training to provide holistic care.

New CagL Amino Acid Polymorphism Patterns of Helicobacter pylori in Peptic Ulcer and Non-Ulcer Dyspepsia

Caliskan, Reyhan; Polat Sari, Silva; Ercan, Bahadir; Peker, Kivanc Derya; Omac Sonmez, Mehtap; Akgul, Ozer; Sapmaz, Burcu; Soylu, Aliye; Adas, Gokhan Tolga; Oner, Yasar Ali

Background and Objectives: Helicobacter pylori infection is associated with chronic gastritis, ulcers, and gastric cancer. The H. pylori Type 4 secretion system (T4SS) translocates the CagA protein into host cells and plays an essential role in initiating gastric carcinogenesis. The CagL protein is a component of the T4SS. CagL amino acid polymorphisms are correlated with clinical outcomes. We aimed to study the association between CagL amino acid polymorphisms and peptic ulcer disease (PUD) and non-ulcer dyspepsia (NUD). Materials and Methods: A total of 99 patients (PUD, 46; NUD, 53) were enrolled and screened for H. pylori by qPCR from antrum biopsy samples. The amino acid polymorphisms of CagL were analyzed using DNA sequencing, followed by the MAFFT sequence alignment program to match the amino acid sequences. Results: Antrum biopsy samples from 70 out of 99 (70.7%) patients were found to be H. pylori DNA-positive. A positive band for cagL was detected in 42 out of 70 samples (PUD, 23; NUD, 19), and following this, these 42 samples were sequenced. In total, 27 different polymorphisms were determined. We determined three CagL amino acid polymorphism combinations, which were determined to be associated with PUD and NUD. Pattern 1 (K35/N122/V134/T175/R194/E210) was only detected in PUD patient samples and was related to a 1.35-fold risk (p = 0.02). Patterns 2 (V41/I134) and 3 (V41/K122/A171/I174) were found only in NUD patient samples and were linked to a 1.26-fold increased risk (p = 0.03). Conclusions: We observed three new patterns associated with PUD and NUD. Pattern 1 is related to PUD, and the other two patterns (Patterns 2 and 3) are related to NUD. The patterns that we identified include the remote polymorphisms of the CagL protein, which is a new approach. These patterns may help to understand the course of H. pylori infection.

Dundar, Tolga Turan; Yurtsever, Ismail; Pehlivanoglu, Meltem Kurt; Yildiz, Ugur; Eker, Aysegul; Demir, Mehmet Ali; Mutluer, Ahmet Serdar; Tektaş, Recep; Kazan, Mevlude Sila; Kitis, Serkan

Objectives: Artificial intelligence (AI) applications in neurosurgery have an increasing momentum as well as the growing number of implementations in the medical literature. In recent years, AI research define a link between neuroscience and AI. It is a connection between knowing and understanding the brain and how to simulate the brain. The machine learning algorithms, as a subset of AI, are able to learn with experiences, perform big data analysis, and fulfill human-like tasks. Intracranial surgical approaches that have been defined, disciplined, and developed in the last century have become more effective with technological developments. We aimed to define individual-safe, intracranial approaches by introducing functional anatomical structures and pathological areas to artificial intelligence. Methods: Preoperative MR images of patients with deeply located brain tumors were used for planning. Intracranial arteries, veins, and neural tracts are listed and numbered. Voxel values of these selected regions in cranial MR sequences were extracted and labeled. Tumor tissue was segmented as the target. Q-learning algorithm which is a model-free reinforcement learning algorithm was run on labeled voxel values (on optimal paths extracted from the new heuristic-based path planning algorithm), then the algorithm was assigned to list the cortico-tumoral pathways that aim to remove the maximum tumor tissue and in the meantime that functional anatomical tissues will be least affected. Results: The most suitable cranial entry areas were found with the artificial intelligence algorithm. Cortico-tumoral pathways were revealed using Q-learning from these optimal points. Conclusions: AI will make a significant contribution to the positive outcomes as its use in both preoperative surgical planning and intraoperative technique equipment assisted neurosurgery, its use increased.

Duru, Özlem Ateş; Araç, Fatma Özsel Özcan; Aldemir, Ozan

Rocket salad (Eruca .sativa), a member of the Brassicaceae family, is an important vegetable for human health because of its antioxidant, anti-inflammatory, anticancer, antiproliferative, and antiangiogenesis properties, as well as its rich chemical composition. Important phytochemical substances in rocket salad, including flavonoids and glucosinolates, have an important function in human health protection. These components can scavenge free radicals, reduce lipid peroxidation, and have anticancer effects by activating apoptosis in cancer cells. As a result, in terms of defending human health, it is believed that the consumption of this vegetable has a significant role in protecting against diseases caused by numerous factors and in the healthy functioning of the body's immune system.

Stability and release of bioactives from liposomes

    Rauf, Muhammed Abdur

The health benefits of bioactive compounds have been proved beyond doubt, but their instabilities and poor bioavailabilities most often limit their full potential. Encapsulating them in vesicular carrier systems, such as liposomes or emulsions, protects the sensitive compounds against damaging factors and offers an opportunity to achieve controlled release, which in turn aims to enhance their bioavailabilities and efficacies. Liposomes and emulsions are regularly used to tackle the stability issues of bioactives, however, they themselves are susceptible to various chemical, physical, and biological instabilities. Different factors related to lipids, bioactives, and liposome size and lamellarity are known to affect both the stabilities and release behavior of liposomes. Liposomes show biphasic or triphasic release patterns with an initial burst release which is brought under control to achieve better release properties. Optimizing release-affecting factors, utilization of the bioactive–cyclodextrin complex, polymeric coating of liposomes, or encapsulation of liposomes into another polymeric matrix are reported to control the liposomal release behavior. Moreover, liposomes are designed to hold (slow or no release) their content until stimulated/triggered by some release-triggering mechanism like pH, temperature, ultrasound, etc. In comparison, double/multiple emulsion and polyelectrolyte coated multilayer emulsions can also offer controlled release of their payload. Liposomes, being made of natural ingredients, are an attractive choice as carriers for bioactives, although some challenges remain. The first is the destruction of liposomes by bile and lipases in the gastrointestinal tract, the most plausible route of administration for bioactives. In addition, they do not permeate through the gastrointestinal epithelial layers. A large amount (often 20%–30%) of cholesterol, a common ingredient required for liposome rigidity, also remains a concern for hypercholesterolemic people. However, our intense interest in bioactive substances can be expected to overcome these challenges in the near future.

The effect of Apfel risk score and fasting times on postoperative nausea and vomiting: Post-Operative Nausea and Vomiting

Şişman, Hamide; Arslan, Sevban; Gezer, Derya; Akpolat, Refiye; Alptekin, Dudu; Gökçe, Esma

Background: Postoperative nausea and vomiting (PONV) is an undesirable problem with many adverse effects on the patient. The first step toward effective treatment and prevention is to identify the patient's risk factors for PONV. This descriptive, cross-sectional study was conducted in the General Surgery Clinic between August 2017 and August 2018 to evaluate the effect of Apfel risk score and fasting duration on PONV and draw attention to this issue. Methods: A total of 283 patients who were scheduled for elective surgery were included in the study. The patients were evaluated using the Patient Evaluation Form created by the researcher by scanning the literature, and the Apfel Risk Scoring System. Results: The solid fasting time of the patients was 18.9 ± 28.9 h, the mean fluid fasting time was 13.9 ± 7.4 h, the mean operation time was 133 ± 76 min, the perceived hunger score was 3.1 ± 3.2, and the mean thirst score was 4.3 ± 3.2 and 16.8 ± 18.5. When the Apfel Risk Score and the incidence of all-time postoperative nausea and vomiting (PONV) were compared, a significant correlation was found between the patients' solid food fasting times and the incidences of the 2nd, 4th, 8th, and 24th hours. PONV incidence (p ˂ 0.01). Conclusion: The fasting times of the patients are longer than recommended. There is a parallel relationship between both long fasting times and the Apfel risk score and the incidence of PONV. It is recommended that institutions establish an evidence-based policy and take the necessary precautions in the preoperative evaluation of patients in terms of postoperative nausea and vomiting and during fasting periods.

Anxiety and Depression After Cesarean: Non-pharmacological Evidence Based Practices; [Sezaryen Sonrası Kaygı ve Depresyon: Non-Farmakolojik Kanıt Temelli Uygulamalar]

Terzioğlu, Füsun; Gençbaş, Dercan; Boztepe, Handan; Doğu, Nilgün; Akdeniz, Canberk; Yüceer, Buğse

The purpose of this review is to examine the non-pharmacological evidence-based practices of anxiety and depression after a cesarean section delivery. Postpartum anxiety and depression have increased in the last decade, the rates of postpartum depression are around 13% worldwide. In the postpartum period, women may experience anxiety and depression due to the operation they have undergone; such as being in an unfamiliar environment, facing new technological equipment and encounters with the medical teams, the post-operative pain, the new roles women undertake as mothers. One of the factors that increase anxiety and depression in the postpartum period is the type of delivery. In Turkey, the rate of cesarean delivery is comparatively high, which is approximately 52%. Anxiety and depression after cesarean section develop due to fear and concern such as the complications that may occur during and after the mother's anesthesia, the possibility delaying breastfeeding her baby, and experiencing pain. Non-pharmacological evidence-based applications such as reiki, acupressure, hand and foot massage, yoga, reflexology, aromatherapy, skin to skin care, nursing care protocols were found to be effective in studies conducted to reduce anxiety and depression after cesarean-section. Nurses who have critical roles and responsibilities in pre-and post-cesarean care practices are recommended to include these evidence-based non-pharmacological practices in routine care practices.

Multiple dens invaginatus in Wilson's disease: A case report

    Memis, Sadi; Bas, Zubeyir

Wilson's disease (WD) is a rare autosomal recessive genetic disease that affects copper metabolism. Anomalies can be seen in the dento-maxillofacial structures of WD patients. Dens invaginatus (DI) is an uncommon tooth anomaly, and its incidence in decidious and permanent molars is even lower. This case report primarily explored the multiple DI of a patient with WD. A 9-year-old boy was admitted to our clinic with complaints of pain and swelling in the right lower molar area. It was learnt that the patient was diagnosed with WD after he was born. Fistula, submandibular lymphadenopathy and diffuse swelling were detected. Using cone-beam computed tomography (CBCT), multiple DIs were observed in bilateral bimaxillary four first molars and deciduous molars. When the patient's systemic condition was considered, extraction was planned under oral antibiotic therapy and was performed. During the 6-month follow-up, uneventful healing was observed.

Incidence of Neuropathic Pain in Patients with Thoracic Disc Pathologies Presenting with Chronic Upper Back Pain and Correlation of Herniation Grade with Pain Severity

    Ahi, Emine Dundar; Goktan, Asli

AIM: To determine the incidence of neuropathic pain (NP) in patients with thoracic disc herniation (TDH) and to determine whether there is any correlation between pain severity and herniation grade. MATERIAL and METHODS: A total of 110 patients diagnosed with TDH with chronic non-specific thoracic spine pain were included in our cross-sectional observational study. Data including magnetic resonance imaging findings, bulging, protrusion, extrusion and sequestration were retrospectively analysed. We determined the incidence of NP in patients by administering the self report form Douleur Neuropathique 4 (DN4) questionnaire. Pain severity was assessed using Numeric Rating Scale scores. RESULTS: In our study, 79.1% of TDHs were located below the T7-8 level. The patients were divided into two subgroups on the basis of the results of the DN4 questionnaire: NP subgroup, 43 (39.1%), and non-NP subgroup, 67 (60.9%). Although there was no statistically significant association between presence of NP and herniation grade bulging, protrusion or sequestration (p>0.05), the rate of herniation in the extrusion grade was statistically significantly higher in patients with NP than in those without NP (p=0.004). The pain severity of the patients with herniation grade extrusion was statistically higher than that of patients with other non-extrusion grades (p=0.035). CONCLUSION: TDH should be considered in patients with neuropathic chronic back, chest and abdominal pain. If the patient's pain characteristics indicate NP, a diagnosis of TDH becomes more likely, considering that nearly 4 out of 10 patients with TDH have NP.

Detection of methicillin resistant staphylococcus aureus strains and typing of staphylococcal cassette chromosome mec from various foods originated different region from turkey; [Türkiye’nin farklı bölgeleri kaynaklı toplanan çeşitli gıda maddelerinden metisilin dirençli staphylococcus aureus suşlarının tespiti ve stafilokokkal kaset kromozom mec tiplendirilmesi]

    Issa, Ghassan; Aydin, Ali

Staphylococcus aureus is a microorganism that is highly resistant to environmental conditions and is widely found in environmental sources. It can cause a large number of infections in both humans and animals. Resistance to methicillin in S. aureus strains occurs due to the production of low affinity penicillin binding proteins (PBP2a). PBP2a is encoded by the mecA gene. The mecA gene is located on a mobile, large genetic element called the staphylococcal cassette chromosome mec (SCCmec). Until now, 13 SCCmec (I-XIII) types have been identified in S. aureus strains. In this study, in different 7 regions (Marmara, Aegean, Central Anatolia, the Black Sea, the Mediterranean, Eastern and Southeastern Anatolia) of Turkey obtained from a variety of points of 700 food items in the [(dairy products (n:560), bakery products (n:89), ready meal (n:40), meat product (n:11)], after the isolation of cultural S. aureus and verification by PCR, MRSA detection and SCCmec typing were aimed. 67 (9.57%) S. aureus strains were isolated from 700 food samples analyzed within the scope of the study. Only 1 (0.14%) of the 67 S. aureus strains isolated, both phenotypically and genotypically, was found to be MRSA and when SCCmec was typed, it was found to be Type IV. Community-acquired MRSA strains can cause clinical cases ranging from skin infections to fatal pneumonia and sepsis, as well as foodborne diseases. As a result, it is considered that MRSA strains can be an important source of contamination for humans with the consumption of food of animal origin.

Comparative clinical study with a novel technique for stabilization of the nasal tip projection and rotation: fixation of the septum and the strut graft through the septal cartilage autoextension

Bilgili, Ahmet Mert; Özkan, Aret Cerci

Purpose It is important to reinforce nasal tip support for long-term satisfactory results. Two workhorses of tip support are strut grafting and the tongue-in-groove technique. Tongue-in-groove causes retraction and stiffness; the strut causes loss of rotation and projection of tip. We introduced a novel technique that avoids these consequences. Methods Between February 2018-February 2019, 40 female underwent primary rhinoplasty. Mean age: 27 years (18-41 years). Follow-up:1-2 years. In first group (20 patients), caudal septum was resected and strut was not fixated to caudal septum. In second group (20 patients), tongue-like extension (septal autoextension) was preserved at caudal septum and sutured to strut. Preoperative, peroperative, and postoperative images were compared. Results Tip projection ratio calculations, nasolabial angle measurements were evaluated by statistical analyses. The change in projection ratios between peroperative period and 1-year postoperative period of strut-only group was higher than septal autoextension group (p = 0.001). This result indicates that nasal tip projection loss in strut-only group was higher compared to septal autoextension group. The change in nasolabial angle measurements from peroperative period to 1-year postoperative period differed significantly between two groups (p = 0.001). Significant difference was observed between 1-year postoperative nasolabial angle measurements of two groups (p = 0.006); septal autoextension group measurements were higher than strut-only group. These results showed that fixation of strut graft to the septal autoextension provides better preservation of the nasolabial angle and stabilization of nasal tip projection. Conclusion Fixation of septum and strut graft through septal autoextension provides satisfactory stabilization of tip projection and rotation without stiffness or rigidity.

Examination of Turkey's health system efficiency

    Ocelik, Mail; Yigit, Pakize

Purpose: The rapid increase in the share of health expenditures in recent years and the efforts of the countries to use the limited resources efficiently have been the essential factor that accelerated the efficiency studies at the macro level in the health sector. This research was conducted to Turkey to examine the efficiency of the health system at the provincial level (NUTS-3). This study also investigates sources of inefficiency and the factors influencing that efficiency in Turkey. Materials and Methods: In the research, Data Envelopment Analysis (DEA) for efficiency and multivariate Tobit regression analysis were used to determine the factors related to efficiency. Efficiency analysis were performed in the study with input-oriented CCR, BCC and RTS methods. In the analysis for each province is taken total hospital beds (per 1 000 population), physicians density (per 1 000 population), nurses density (per 1 000 population) as an input and infant mortality (per 1000 live births), and life expectancy at birth as output. Results: According to the research findings, 10 provinces were found to be total efficiency (CCR) and 14 provinces were technically efficiency (BCC). The average total efficiency score of the provinces was found to be 0.8051 (CCR), and the technical efficiency score was 0.8332 0,8332 is 0.9649. Conclusion: The inefficiency in the inputs of the provinces is partly due to the inefficiency of scale, while the technical inefficiency is largely due to resource use. The research findings provided information to managers and policymakers about the technical efficiency of the provinces and the changes to be made in health inputs to make the inefficient provinces relatively efficient.

Is arthrocentesis plus hyaluronic acid superior to arthrocentesis alone in the treatment of disc displacement without reduction in patients with bruxism?

    Hosgor, Hatice

Purpose: This study was designed to investigate the efficacy of arthrocentesis with and without intra-articular injection of hyaluronic acid (HA) in the treatment of disc displacement without reduction (DDwoR) in patients with bruxism. Materials and methods: A retrospective cohort study was conducted using the files of patients with DDwoR, limited mouth opening, and bruxism. Patients were divided into two groups according to the treatment method: arthrocentesis alone (group 1) and arthrocentesis plus intra-articular HA injection (group 2). To evaluate the clinical outcomes for the groups, maximum mouth opening (MMO) and visual analog scale (VAS) values before treatment, immediately after the operation, and at 1, 3, 6, 9, and 12 months postoperatively were analyzed. Results: The study consisted of 40 patients (34 female and 6 male, mean age 36.0 +/- 11.95 years). MMO values increased and VAS scores decreased in both groups at all follow-ups after the treatment compared with pretreatment values (p < 0.001). The increase in MMO in group 2 was greater than in group 1 only at 3 and 6 months postoperatively (p = 0.013 and 0.009, respectively). Conclusions: Intra-articular HA injection is not a relevant addition when arthrocentesis is performed and a stabilizing splint is used. Detection of Methicillin-Resistant Staphylococcus aureus in Milk by PCR-Based Phenotyping and Genotyping

    Issa, Ghassan; Aksu, Harun

Staphylococcus aureus is a primary cause of food poisoning and many types of other infections in humans and animals. This study aims to detect methicillin-resistant S. aureus (MRSA) in raw cow milk collected from different farms in Istanbul, using polymerase chain reaction–based phenotyping and genotyping. A total of 406 raw milk samples were collected from 6 different dairy farms located in Istanbul at different times and under appropriate conditions. The samples were transferred to laboratory under cold storage conditions. After S. aureus had been isolated from the samples by phenotypic methods, MRSA strains were detected in isolates by both phenotypic and genotypic methods. Of the 406 milk samples examined, S. aureus strains were detected in 119 (29.31%). From the 119 isolated S. aureus strains, there was merely 1 (0.84%) strain with MRSA in phenotypic and genotypic terms. Strategies required to control antimicrobial resistance should be developed and implemented in the field of veterinary and human medicine.

Molecular signatures of chronic periodontitis in gingiva: A genomic and proteomic analysis

Guzeldemir-Akcakanat, Esra; Alkan, Begum; Sunnetci-Akkoyunlu, Deniz; Gurel, Busra; Balta, V. Merve; Kan, Bahadir; Akgun, Emel; Yilmaz, Elif Busra; Baykal, Ahmet Tarik; Cine, Naci

Background To elucidate molecular signatures of chronic periodontitis (CP) using gingival tissue samples through omics-based whole-genome transcriptomic and whole protein profiling. Methods Gingival tissues from 18 CP and 25 controls were analyzed using gene expression microarrays to identify gene expression patterns and the proteins isolated from these samples were subjected to comparative proteomic analysis by liquid chromatography-tandem mass spectrometry (LC-MS/MS). The data from transcriptomics and proteomics were integrated to reveal common shared genes and proteins. Results The most upregulated genes in CP compared with controls were found as MZB1, BMS1P20, IGLL1/IGLL5, TNFRSF17, ALDH1A1, KIAA0125, MMP7, PRL, MGC16025, ADAM11, and the most upregulated proteins in CP compared with controls were BPI, ITGAM, CAP37, PCM1, MMP-9, MZB1, UGTT1, PLG, RAB1B, HSP90B1. Functions of the identified genes were involved cell death/survival, DNA replication, recombination/repair, gene expression, organismal development, cell-to-cell signaling/interaction, cellular development, cellular growth/proliferation, cellular assembly/organization, cellular function/maintenance, cellular movement, B-cell development, and identified proteins were involved in protein folding, response to stress, single-organism catabolic process, regulation of peptidase activity, and negative regulation of cell death. The integration and validation analysis of the transcriptomics and proteomics data revealed two common shared genes and proteins, MZB1 and ECH1. Conclusion Integrative data from transcriptomics and proteomics revealed MZB1 as a potent candidate for chronic periodontitis.

The relationship between temporomandibular joint effusion and pain in patients with internal derangement

    Hosgor, Hatice

Objectives: The purpose of this study was to evaluate the relationship between temporomandibular joint (TMJ) effusion and joint pain in patients with internal derangement based on the Diagnostic Criteria for Temporomandibular Disorders (DC/TMD). Methods: A total of 240 TMJs from 120 patients with unilateral painful joints (103 females and 17 males, mean age 29.9 +/- 12 years) were evaluated. Clinical assessments were carried out according to the DC/TMD guidelines. Magnetic resonance imaging (MRI) was used to evaluate the degree of effusion in each joint. The radiological and clinical findings were analysed for statistically significant correlations. Results: Although the results indicated a statistically significant association between moderate joint effusion and disc displacement (p < 0,05), there was no statistically significant association between moderate effusion and joint pain (p > 0,05). There were, however, statistically significant associations between marked effusion and both disc displacement and joint pain (p < 0,05). Conclusion: TMJ effusion is associated with both disc displacement and joint pain: the effusion increased in direct proportion to the severity of pain and disc displacement. The possibility that there are various aetiologies for the condition should also be considered. (C) 2019 European Association for Cranio-Maxillo-Facial Surgery. Published by Elsevier Ltd. The purpose of this study was to compare the incidence and longitudinal propagation of dentin defects after gutta-percha removal with hand and rotary instruments using microcomputed tomography. Twenty mandibular incisors were prepared using the balanced-force technique and scanned in a 19.9 mu m resolution. Following filling with the lateral compaction technique, gutta-percha was removed with ProTaper Universal Retreatment (PTUR) or hand instruments. After rescanning, a total of 24,120 cross-sectional images were analyzed. The numbers, types, and longitudinal length changes of defects were recorded. Defects were observed in 36.90% of the cross sections. A total of 73 defects were comprised of 87.67% craze lines, 2.73% partial cracks, and 9.58% fractures. No significant difference in terms of new defect formation was detected between the retreatment groups. The apical and middle portions of the roots had more dentin defects than the coronal portions. Defects in three roots of the PTUR instrument group increased in length. Under the conditions of this in vitro study, gutta-percha removal seemed to not increase the incidence of dentin defect formation, but the longitudinal defect propagation finding suggests possible cumulative dentinal damage due to additional endodontic procedures. Hand and rotary instrumentation techniques caused similar dentin defect formation during root canal retreatment.
