Smart Nanocarriers and AI-Driven Release Systems for Targeted Antibiotic Delivery

Kostas, Rabia Yasa; Bozbeyoglu, Pinar; Altuner, Elif Esra; Elmas, Muzaffer

The antimicrobial resistance (AMR) crisis is not merely a consequence of the emergence ofresistant pathogens. Furthermore, the remaining shortcomings oftraditional antibiotic production and development processes have become a profound health threat, exacerbated by their pharmaceutical and pharmacokinetic limitations. This book chapter redefines the AMR crisis as a problem of drug and treatment discovery as well as drug delivery to target tissues, and examines the technological angle emerging at the intersection of nanotechnology and artificial intelligence (AI) in response to this problem. This review begins by examining the problems inherent in traditional antibiotic architecture, such as systemic toxicity, low bioavailability, poor stability, and, most importantly, the inability to penetrate protected sites such as bacterial biofilms or intracellular bunkers. Alternative diagnoses in neonates undergoing therapeutic hypothermia for presumed hypoxic–ischemic encephalopathy

Donmez, Tugrul; Serce Pehlevan, Ozge; Gunlemez, Ayla

Background – Early identification of hypoxic–ischemic encephalopathy (HIE) remains challenging during the first hours of life, and therapeutic hypothermia (TH) is often initiated before the underlying diagnosis can be established. Objective – The primary aim of this work is to compare the clinical characteristics, laboratory findings, and short-term outcomes of neonates treated with TH for presumed HIE according to their predominant underlying diagnosis. Methods – We retrospectively reviewed all neonates who received TH for suspected HIE between 2019 and 2023. Infants were categorized according to the predominant underlying diagnosis identified following comprehensive clinical, neurophysiological, neuroimaging, metabolic, genetic, and subspecialty evaluations. Demographic, clinical, neurologic, and laboratory parameters—including inflammation- and oxidative stress-related indices—were compared between groups. Results – Among 92 neonates treated with TH, nine (10%) were subsequently found to have non-hypoxic–ischemic etiologies of neonatal encephalopathy. Baseline neurological severity assessed by Sarnat staging and initial amplitude-integrated electroencephalography patterns did not differ between groups. However, infants in the non-HIE group demonstrated significantly higher rates of myocardial injury (77% vs. 22%, p < 0.001) and severe thrombocytopenia (66% vs. 25%, p = 0.016). These infants also had substantially longer hospitalizations [median (25P–75P): 28 (17–38) vs. 7 (5–11) days, p = 0.002] and higher mortality rates (55.6% vs. 6%, p = 0.001). The median day of death was earlier in the non-HIE group [median (25P–75P): 3 (1–57) vs. 16 (8–77) days; p = 0.30]. However, it was not statistically significant. Inflammatory and oxidative stress indices did not differ significantly between groups. Conclusion – Approximately 10% of neonates undergoing therapeutic hypothermia for presumed HIE were ultimately found to have non-hypoxic–ischemic causes of neonatal encephalopathy despite similar early neurologic presentations. These infants experienced greater systemic morbidity and poorer short-term outcomes. While the small and heterogeneous non-HIE cohort warrants cautious interpretation, disproportionately severe systemic manifestations—particularly myocardial injury and thrombocytopenia—may represent early clinical clues prompting broader diagnostic evaluation beyond hypoxic–ischemic injury.

Therapeutic potential of Artemisia Kurramense against Helicobacter pylori infection-associated gastric pathogenesis: A network pharmacology, molecular docking, and DFT study

Alkholifi, Faisal K.; Ali, Nijat; Foudah, Ahmed I.; Ahmad, Muhammad; Yusufoglu, Hasan Soliman; Jamil, Shahid

Introduction: Artemisia Kurramense has been used for gastrointestinal and inflammatory conditions, but no attempt has been made to explore the possible computational molecular mechanism against Helicobacter pylori-associated gastric pathogenesis. This study aimed to develop a computational hypothesis of the therapeutic potential of A. Kurramense by combining the phytochemical profiling based on LC-HRMS with network pharmacology, molecular docking and density functional theory (DFT) analysis. Methods: LC-HRMS profiling of three extracts of A. Kurramense resulted in the identification of 40 phytochemicals, of which 37 were unique across all three extracts analyzed. Oral bioavailability and drug-likeness screening were carried out for the identified compounds, which were then subjected to target prediction, disease-associated target intersection, protein–protein interaction (PPI) network analysis, Gene Ontology and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment analysis, molecular docking study and density functional theory (DFT) calculation. Results: Integrated analysis identified 131 common therapeutic targets, with AKT1, PIK3CA, and EGFR as prominent hub targets. Functional enrichment identified the PI3K-Akt pathway as the top enriched pathway, followed by cancer pathways, HIF-1, FoxO, MAPK and apoptosis pathways. Key regulatory proteins in the PI3K-Akt network were AKT1, PIK3CA, EGFR, PIK3R1 and TP53. The prioritized phytochemicals showed good predicted binding affinity with AKT1, with the greatest affinity for quercetin (AK-7) (−10.047 kcal/mol). At the same time, the DFT analysis revealed favourable electronic properties and chemical reactivity of the prioritized compounds. Conclusion: The computational results support a hypothesis that A. Kurramense is a multi-target modulator of host inflammatory, survival, oxidative-stress, and apoptosis-related signaling pathways, with the PI3K-Akt signaling pathway as a potential central mechanism of action. Some prioritized phytochemicals, such as quercetin, are promising for further investigation. However, these are computational predictions and further studies to validate the anti-H. pylori activity, target engagement, pathway modulation, and gastric protective effects require appropriate in vitro and in vivo studies.

Predictive factors associated with the safety and efficacy of refractive surgery for Myopia: a retrospective cohort study

    Yenihayat, Fatih; Sumer, Fatma

Background/Objectives: Wavefront-guided refractive surgery delivers excellent visual outcomes for myopia, but the relative contribution of demographic, refractive, and corneal preoperative variables to safety and efficacy remains incompletely characterized in real-world single-surgeon cohorts. We aimed to identify independent predictors of safety and efficacy after wavefront-guided femtosecond LASIK (i-LASIK) and wavefront-guided PRK (wfPRK) for myopia, using a methodologically robust analytical approach. Methods: Single-center, single-surgeon retrospective cohort of consecutive myopic patients operated between January 2020 and July 2024. After exclusion of hyperopia, ectatic disorders, incomplete follow-up, and amblyopia (preoperative best-corrected visual acuity [BCVA] logMAR ≥ 0.3), 329 patients (648 eyes) were analyzed. The primary outcomes were the safety index (postoperative BCVA / preoperative BCVA) and the efficacy index (postoperative uncorrected visual acuity / preoperative BCVA), modelled as continuous variables. To preserve the independence assumption while making full use of bilateral data, all inferential analyses — Spearman correlations, Mann–Whitney U / Kruskal–Wallis H tests, and multivariable linear regression with eight preoperative predictors entered simultaneously — were performed separately for right and left eyes. Standardized regression coefficients (β) and 95% confidence intervals are reported. Results: The cohort included 162 male (49.2%) and 167 female (50.8%) patients, mean age 30.0 ± 6.4 years; 411 eyes (63.4%) underwent i-LASIK and 237 (36.6%) wfPRK. Median follow-up was 101 days (range 23 days to 4.2 years). Overall, 99.4% of eyes had a safety index above 0.85 and 98.8% had an efficacy index above 0.80. Across all four multivariable models (safety and efficacy, right and left eyes), preoperative BCVA logMAR was the strongest independent predictor (β = +0.437 to +0.682, all p < 0.001). Preoperative astigmatism was independently associated with reduced left eye safety (β = −0.102, p = 0.028), and high myopia (SEQ ≤ −6.01 D) with reduced right eye safety (β = −0.096, p = 0.040) and efficacy (β = −0.113, p = 0.043). Age, sex, and surgical technique (i-LASIK vs wfPRK) were not independently associated with either index (all p > 0.05). Conclusions: In a real-world wavefront-guided refractive surgery cohort, both i-LASIK and wfPRK delivered comparably high safety and efficacy. The strongest determinant of both indices was preoperative BCVA logMAR, while preoperative astigmatism and high myopia showed small but consistent independent contributions. The absence of an independent age, sex, or technique effect supports an indication-based rather than procedure-based decision framework and a more nuanced view of refractive surgery outcome prediction.

Circulating Ghrelin, Nesfatin-1, and Vaspin Levels Across Glycemic Control Categories in Type 2 Diabetes Mellitus: A Cross-Sectional Study

Akkus, Suheda; Saricam, Orkun; Kavruk, Murat; Sariyildiz, Gulcin Turkmen; Benguboz, Fatma Busra; Sirin, Berna Yilmaz; Altuner, Elif Esra; Dursun, Ali Dogan; Ozalp, Veli Cengiz

Background/Objectives: Diabetes mellitus is characterized by chronic hyperglycemia, insulin resistance, and low-grade inflammation, and adipose-tissue-derived adipokines are increasingly implicated as regulators of glucose homeostasis. This study compared circulating ghrelin, nesfatin-1, and vaspin levels across three glycemic-control categories defined by HbA1c and evaluated their association with glycemic status. Methods: In this cross-sectional comparative study, serum ghrelin, nesfatin-1, and vaspin were measured by enzyme-linked immunosorbent assay (ELISA) in 161 adults stratified into three groups: HbA1c 6.0–8.4% (n = 55), HbA1c ≥ 8.5% (n = 52), and non-diabetic controls with HbA1c < 6.0% (n = 54). Distributional assumptions were tested with the Kolmogorov–Smirnov and Shapiro–Wilk tests. Because all three adipokines deviated from normality, the primary omnibus comparison was the Kruskal–Wallis test, followed by Dunn’s post hoc test with Bonferroni adjustment; effect sizes are reported with 95% confidence intervals. Group differences were additionally examined after adjustment for age and sex, and HbA1c was analysed as a continuous variable. Results: Ghrelin (Kruskal–Wallis p = 0.726) and vaspin (p = 0.364) did not differ among the three groups. Nesfatin-1 differed significantly (H = 15.16, p < 0.001, ε2 = 0.095) and non-monotonically: concentrations were higher in the HbA1c 6.0–8.4% group than in controls (median 824 versus 650 pg/mL; Dunn p = 0.018) and than in the HbA1c ≥ 8.5% group (824 versus 600 pg/mL; p < 0.001), whereas the HbA1c ≥ 8.5% and control groups did not differ (p = 0.909). The nesfatin-1 difference persisted after adjustment for age and sex (p < 0.001) and after excluding controls with impaired fasting glucose (p < 0.001). Within the diabetic participants, nesfatin-1 correlated inversely with continuous HbA1c (ρ = −0.373, p < 0.001). Conclusions: Nesfatin-1, but not ghrelin or vaspin, differed significantly between HbA1c-defined glycemic-control categories in a non-monotonic manner, with the highest concentrations in the intermediate-HbA1c group. Because body mass index, diabetes duration, and antidiabetic medication were not available, and because the design is cross-sectional, these between-group differences cannot be interpreted as a within-person trajectory or attributed to glycemic status independently of adiposity. These exploratory findings require confirmation in larger, prospective, BMI- and medication-adjusted cohorts before nesfatin-1 can be considered a candidate biomarker.

The use of comfort theory in the intensive care unit

    Koç, Ferhat; Dikmen, Yurdanur

Background: A well-functioning intensive care unit should support the patient's recovery by promoting their participation in treatment and care. Enhancing patient comfort in the intensive care unit can maximise the quality of nursing care and shorten the recovery period. Aim: To evaluate the effects of nursing interventions based on comfort theory on comfort levels and perceived environmental stressors of patients in the intensive care unit. Methods: A randomised controlled trial was conducted between 15 October 2019 and 15 February 2020, including 50 intensive care unit patients divided into intervention (n=25) and control (n=25) groups. The intervention group received structured nursing care plans designed according to comfort theory, while the control group received standard intensive care unit care. Data collection tools included the Patient Identification Form, General Comfort Scale and Intensive Care Unit Environmental Stressor Scale. Data analysis used parametric tests to assess differences between groups. Results: The intervention group had a significantly higher comfort level (General Comfort Scale score: 142±28±10.84) compared to the control group (General Comfort Scale score: 111.36±13.34). Additionally, the Intensive Care Unit Environmental Stressor Scale scores, which indicate perceived stressors, were lower in the intervention group (86.25±14.38) compared to the control group (111.80±14.25), suggesting that structured nursing care significantly reduced environmental stress. Conclusions: Findings demonstrated that nursing interventions structured according to comfort theory improved patient comfort levels and reduced environmental stressors in intensive care unit patients. The study highlights the importance of patient-centered nursing care and technology-assisted interventions in enhancing patient wellbeing in critical care settings. Further research is recommended to explore the long-term benefits of these interventions in diverse intensive care unit populations.

Development of Fluorimetric Method for Controlled Drug Release From Vinyl Propionate-co-Maleic Anhydride Polymer Modified With Paracetamol

Memis, Yilmaz; Gunduz, S. Beniz; Altuner, Elif Esra; Tatli, Havva Nur; Ozen, Gurkan; Okudan, Ahmet; Civil, Serife; Dar, Umar Ali

A novel pH-responsive polymeric drug delivery system is developed by covalently binding the active pharmaceutical ingredient paracetamol (PAR) to a synthesized vinyl propionate-co-maleic anhydride (VPR-co-MA) copolymer. The controlled release of PAR from the resulting VPR-co-MA-PAR conjugate was investigated in vitro at various physiological pH values (2.0, 4.5, 7.4, and 9.0) at 37°C over a 6 h-period. A fluorimetric method was utilized to quantify the released PAR, with optimal excitation and emission wavelengths determined to be 330 and 433 nm, respectively. The results demonstrated a distinct pH-dependent release profile. After 6 h, the cumulative drug release was significantly suppressed at acidic gastric pH 2.0 (42.23%), whereas it was substantially higher at neutral and alkaline conditions, reaching 71.29% at pH 7.4 and a maximum of 74.46% at pH 9.0. The pronounced difference in release rates confirms the system's ability to limit premature drug release in the stomach while promoting targeted delivery in the intestinal tract. This VPR-co-MA-PAR conjugate shows significant promise as a carrier for the controlled oral delivery of paracetamol.

Effect of sacubitril/valsartan and SGLT2 inhibitors on arrhythmia burden in ICD patients

    Öztürk, Fatih; Yaman, Mehmet

Background: This study aimed to examine whether the use of sodium–glucose co-transporter 2 inhibitors (SGLT2i) and/or sacubitril/valsartan in individuals with an implantable cardioverter-defibrillator (ICD) was associated with differences in pathological rhythm burden, specifically atrial fibrillation (AF), ventricular tachycardia (VT), and ventricular fibrillation (VF), during follow-up. Methods: Data from 275 patients who underwent ICD implantation between 2022 and 2025 were analyzed retrospectively. The primary endpoint was change in device-detected arrhythmic burden, categorized as unchanged/increased, 0–50% reduction, 50–100% reduction, or arrhythmia-free status during 12-month follow-up. The secondary endpoint was change in LVEF (ΔEF). Associations between medical therapies and EF change were assessed using multivariable linear regression models adjusted for demographic and clinical covariates. Results: Overall, mean LVEF increased significantly during follow-up (ΔEF 9.7 ± 8.7%, p < 0.001). In adjusted analyses, ARNI use was independently associated with greater EF improvement (B = 10.647, p < 0.001), whereas SGLT2i therapy alone was not significantly associated with ΔEF. In unadjusted analyses, both ARNI and SGLT2i therapies were associated with a higher likelihood of substantial reduction in arrhythmic burden and arrhythmia-free status (p < 0.001 for both). Patients receiving combined ARNI and SGLT2i therapy demonstrated the highest proportion of arrhythmia-free status during follow-up. Conclusion: In this retrospective cohort of ICD recipients with HFrEF, ARNI therapy was independently associated with greater improvement in LVEF, while both ARNI and SGLT2i use were associated with lower device-detected arrhythmic burden in unadjusted analyses. Prospective studies are required to determine causality and clarify the independent antiarrhythmic effects of contemporary guideline-directed therapies in ICD populations.

Clinical Characteristics and Management Outcomes of Pediatric Temporal Bone Fractures: A Retrospective Cohort Study

Bulut, Oktay; Şevik Eliçora, Sultan; Erdem, Duygu

Background: – Pediatric temporal bone fractures may lead to facial paralysis, hearing loss, and cerebrospinal fluid (CSF) leakage. Understanding their clinical and radiologic characteristics is essential for effective management. Methods: – This retrospective, single-center descriptive study included 50 pediatric patients diagnosed with temporal bone fractures between 2013 and 2020. Demographic data, mechanisms of injury, fracture types, facial nerve function, CSF leakage, and audiological findings were analyzed. Fractures were classified according to the McHugh system (longitudinal, transverse, and mixed) and the Kelly-Tami classification [otic capsule sparing (OCS) versus otic capsule violating (OCV)]. Hearing was evaluated using auditory brainstem response (ABR) in children under 6 years and pure-tone audiometry (PTA) with age-appropriate behavioral techniques in older children. Results: – The cohort included 50 patients (mean age: 7.8±3.8 years; 64% boys). Falls were the leading cause of injury, while high-energy traumas such as gunshot and penetrating orbital injuries were rare but associated with complete facial paralysis and total hearing loss. Most fractures were longitudinal and OCS, showing favorable recovery, whereas OCV fractures caused irreversible deficits. CSF leakage occurred in 4 patients, all resolving spontaneously. Surgery was required only for persistent conductive hearing loss, and ossicular reconstruction restored normal hearing. Conclusion: – Pediatric temporal bone fractures are mostly OCS and longitudinal, typically caused by falls. OCV fractures are uncommon but linked to irreversible facial nerve and cochlear injury.

Impact of Adhesive Application Modes on Shear Bond Strength of Resin Composites to Biodentine

Deger, Ceren; Ozduman, Zumrut Ceren; Doğan, Miraç

Purpose: This in-vitro study aimed to evaluate the impact of the adhesive application mode on the shear bond strength of various resin composites to Biodentine. Materials and Methods: A total of 60 acrylic blocks, each with a central hole (2 mm x 5 mm), were made for the study (n = 60). The holes were filled with Biodentine (Septodont), and the samples were randomly divided into three groups based on the type of restorative material used: 1) conventional posterior composite, Estelite Posterior Quick (EP)(Tokuyama); 2) bulk-fill composite, Filtek Bulk Fill (FB) (3M ESPE); and 3) short fiber-reinforced composite, EverX Posterior (EX)(GC). Additionally, each group was subdivided into two categories depending on the adhesive application method: self-etch or total-etch. A universal adhesive (Single Bond Universal, 3M ESPE) was applied to the Biodentine specimens, and subsequently, the resin composites were applied and light-cured for 20 s. The shear bond strength was measured using a universal testing machine (AGS-1000D, Shimadzu) at a crosshead speed of 1 mm/min. The shear bond strength data (MPa) were analyzed using a two-way ANOVA and the Bonferroni test (p < 0.05). Results: For the self-etch application mode, a statistically significant difference was observed among the material groups (p = 0.005). Group EP exhibited higher shear bond strength compared to Group FB. There were also significant differences among the groups for the total-etch application mode (p = 0.009), with Group EP again showing the highest shear bond strength. However, when comparing the two etching modes within each material group, there were no significant differences in shear bond strength. Conclusions: The conventional posterior composite used significantly affects the strength of the shear bond to Biodentine. In contrast, the choice between self-etch and total-etch modes does not notably impact the bond strength.

Objective: This study aimed to evaluate the relationship between metabolic insulin resistance indicators (METS-IR and TyG index) and the amount of epicardial adipose tissue (EAT), with subclinical atherosclerosis markers such as coronary artery calcium (CAC) and carotid intima-media thickness (IMT). Methods: Retrospective data from 108 patients evaluated for suspected coronary artery disease were analyzed. METS-IR and TyG scores were calculated using laboratory and anthropometric data. EAT thickness and volume were measured by computed tomography. CAC score and carotid IMT were assessed using standard methods. Correlations and multivariate regression analyses were used to examine associations, and ROC analysis was performed to evaluate diagnostic performance. Results: Epicardial fat thickness (AUC=0.802) and volume (AUC=0.781) showed high discriminative power in predicting the presence of CAC. A threshold value of 5.9 mm for EAT thickness provided 75% sensitivity and 71% specificity. The METS-IR score was independently associated with both CAC (AUC=0.670) and carotid IMT, while the TyG index showed only a weak association with CAC (AUC=0.661). In multivariate analyses, EAT measurements and METS-IR scores were significantly associated with subclinical atherosclerosis markers, independent of conventional risk factors. Conclusion: METS-IR, TyG, and particularly epicardial fat measurements appear to be potential biomarkers for the early detection of subclinical atherosclerosis. Integration of these parameters into individualized cardiovascular risk assessments should be supported by further studies.

Potential use of quantum imaging and artificial intelligence technologies in neurosurgery

    Turan, Yahya; Caliskan, Ayfer

This study examines the current status and potential future areas of use of artificial intelligence and quantum technologies in neurosurgery within a conceptual framework. Artificial intelligence is widely used, especially in the analysis of imaging data, surgical planning and intraoperative decision support systems, and is rapidly being integrated into clinical practices. Quantum imaging technologies, on the other hand, attract attention with their capacity to provide higher resolution and lower radiation dose, but are currently limited to experimental and pilot studies. The study reveals that artificial intelligence and quantum technologies have complementary properties in neurosurgery; it shows that hybrid approaches such as quantum artificial intelligence can increase clinical success by increasing accuracy, speed and predictive capacity in diagnosis and treatment processes. In addition, it is emphasized that elements such as ethical responsibilities, user education and interdisciplinary collaboration are of critical importance in the clinical integration of these technologies. As a result, artificial intelligence and quantum technologies are seen as important tools that will shape the future surgical practices in neurosurgery.
