- MakaleAçık Erişim2025Factors affecting mortality in cases of perinatal asphyxia treated with hypothermia; A 10-year experience: A retrospective cohort study
Günlemez, Ayla; Dönmez, Tuğrul; Pehlevan, Ozge Serce
Aim: Birth asphyxia and intrapartum brain injury can cause death or irreversible brain damage. The only known treatment that improves prognosis is therapeutic hypothermia. Our goal was to assess the parameters associated with death in newborns receiving therapeutic hypothermia. Materials and Methods: This retrospective cohort study was conducted in a neonatal intensive care unit (NICU) and included neonates who underwent therapeutic hypothermia due to perinatal asphyxia over 10 years. Patients were categorized into survivors and nonsurvivors based on their in-hospital outcomes. Demographic characteristics, clinical parameters, laboratory findings, and complications were retrieved from the patients’ medical records. Prognostic factors were compared between groups using appropriate statistical methods, and multivariate logistic regression was performed to identify independent predictors of mortality. Results: A total of 114 newborns treated with therapeutic hypothermia were analyzed, of whom 11 (9.6%) died due to perinatal asphyxia (PA). Non-survivors had significantly lower gestational ages (37±2 vs. 38.4±1.6 weeks, p=0.042), lower 5-minute Apgar scores (5 [1--5] vs. 5.5 [4--7], p=0.048), and higher cesarean delivery rates (81.8% vs. 41.7%, p=0.011). Severe findings on aEEG and Sarnat Stage III were more common among nonsurvivors (p<0.001 for both). Nonsurvivors also exhibited significantly lower admission rectal temperatures [33.8°C (32--36) vs. 35.8°C (34.5--36), p=0.023], higher rates of thrombocytopenia (81.8% vs. 42.7%, p=0.013), acute renal failure (90.9% vs. 39.8%, p=0.001), and cranial ultrasound abnormalities (27.3% vs. 3.9%, p=0.019). In the multivariate logistic regression analysis, only a lower rectal temperature at admission remained an independent predictor of mortality (OR: 1.520, 95% CI: 1.061--2.178, p = 0.023). Conclusion: Factors such as lower Apgar scores, acute renal failure, and hypothermia severity on admission are strongly associated with mortality. The early identification and management of these risk factors are critical for improving outcomes in neonates treated with therapeutic hypothermia.

Aims: Individuals with severe mental illness (SMI) experience disproportionately poor oral health due to behavioral, pharmacological, and systemic challenges. This study aimed to assess the oral health status of patients with SMI using the oral health assessment tool (OHAT) and the Decayed, Missing, and Filled Teeth (DMFT) Index, and to screen for oral potentially malignant lesions (OPMLs). Methods: A cross-sectional study was conducted with 60 patients registered at the Community Mental Health Center in Kocaeli, Turkiye. Comprehensive clinical examinations were performed using the Turkish version of the OHAT and the DMFT Index. The presence of OPMLs was also recorded. Statistical analyses included descriptive statistics, independent t-tests for gender comparisons, and Pearson correlation analyses to explore associations between age, OHAT, and DMFT scores. Results: Participants (mean age: 47.6±11.5 years; 65% female) had a mean OHAT score of 9.15±2.39 and a mean DMFT score of 13.5±7.00. No significant gender differences were observed in total OHAT or DMFT scores. However, females had significantly more filled teeth than males (3.08±2.04 vs. 1.57±1.80; p=0.006). Age was moderately correlated with OHAT (r=0.318, p=0.013) and strongly correlated with DMFT (r=0.449, p<0.001). OHAT and DMFT scores were also significantly correlated (r=0.502, p<0.001). Fifteen soft tissue lesions were detected, including leukoplakia, lichen planus, and angular cheilitis. Discussion: This study showed a high burden of oral disease and soft tissue lesions among patients with SMI, exceeding levels reported in non-psychiatric and geriatric populations. The findings emphasize the need for interdisciplinary care models incorporating routine dental screening, preventive interventions, and education within mental health services.

- MakaleAçık Erişim2025Metabolic imaging of head and neck lesions: Differentiating dental and maxillofacial conditions using positron emission tomography/computed tomography
Medişoğlu, M.Sabri; Öçbe, Melisa
Aim: Fluorine-18-labeled fluoro-2-deoxy-D-glucose positron emission tomography/computed tomography (FDG-PET/CT) is an essential oncologic imaging modality, but its uptake patterns may also be seen in benign inflammatory or structural oral conditions, leading to diagnostic confusion. This study aims to compare metabolic activity with maximum standardized uptake value (SUVmax) among common dento-maxillofacial conditions---dental artifacts, periodontal infections, maxillary sinus pathologies, and temporomandibular joint (TMJ) disorders---against confirmed malignant lesions, to guide differential diagnosis and improve clinical interpretation. Materials and Methods: A total of 100 FDG-PET/CT scans were retrospectively analyzed. Inclusion criteria comprised pre-treatment scans with available CT or magnetic resonance imaging (MRI) acquired within four weeks. Patients were categorized into five groups: Dental Artifacts (n=19), Periodontal Infections (n=23), Maxillary Sinus Pathologies (n=21), TMJ Pathologies (n=19), and Malignant Dento-maxillofacial Lesions (n=18; control group). All scans were reviewed by an oral and maxillofacial radiologist and a radiologist. SUVmax were calculated using a 42% threshold. Statistical analyses were conducted using Kruskal-Wallis and MannWhitney U tests with Bonferroni correction. Results: Significant differences in SUVmax were observed across all groups (χ² = 77.4, p<0.000001). The malignant lesion group demonstrated the highest metabolic activity (mean SUVmax = 12.5 ± 3.0), significantly greater than all other groups (p<0.000001). Among nonmalignant conditions, maxillary sinus pathologies showed the highest SUVmax (4.2 ± 1.0), followed by periodontal infections (3.5 ± 0.8). TMJ pathologies and dental artifacts exhibited the lowest FDG uptake (2.0 ± 0.6 and 2.5 ± 0.7, respectively), suggesting minimal metabolic activity. Conclusion: This study confirms that malignant dento-maxillofacial lesions exhibit significantly higher FDG uptake compared to inflammatory and mechanical conditions. Understanding these uptake patterns is crucial to avoiding false-positive interpretations, especially in oncologic patients. Multimodal imaging and interdisciplinary evaluation are recommended for accurate differentiation of dental and maxillofacial pathologies in FDG-PET/CT interpretation.

Medişoğlu, M.Sabri; Öçbe, Melisa; Öçbe, Çetin Batuhan
Aims: This study aimed to evaluate and compare temporomandibular joint (TMJ) disc position and condylar morphology on magnetic resonance imaging (MRI) and computed tomography (CT) images of patients with head and neck cancer (HNC) and healthy controls, to determine the prevalence and coexistence of soft-tissue and osseous alterations and to explore their potential clinical relevance. Methods: In this retrospective case-control study, 145 HNC patients and 147 controls that underwent standardized maxillofacial CT and MRI. Anterior disc displacement (ADD) was defined on mid-sagittal MRI; condylar morphology (normal, flattening, erosion, osteophyte) was classified on sagittal CT. Results: Two hundred ninety-two individuals were analyzed (HNC 49.7%). Age was similar (mean difference -3.11 years; p=0.097; Mann-Whitney p=0.0737). ADD prevalence was higher in HNC: right 49.0% vs 37.4% (RD +11.6 percentage points [pp]; RR 1.31; χ² p=0.046), left 52.4% vs 32.7% (RD +19.8 pp; RR 1.61; χ² p=0.00064), and any ADD 60.7% vs 46.9% (RD +13.8 pp; RR 1.29; χ² p=0.018); significance was retained after Holm–Bonferroni adjustment. Condylar morphology distributions differed on both sides (right χ² p=9.6×10-⁵; left p=8.1×10-¹⁰), driven by more erosion in HNC (right 26.9% vs 11.6%; left 27.6% vs 13.6%). “Any condylar pathology” was more frequent in HNC (93.8% vs 85.7%; RD +8.1 pp; OR 2.52; χ² p=0.023; Fisher p=0.033). ADD showed no lateral preference within groups (McNemar p>0.30). Controls, but not HNC patients, exhibited right-greater-than-left discordance for “any condylar pathology” (McNemar p=2.5×10-⁵ vs 1.000). Conclusion: HNC is associated with a high burden of TMJ abnormalities; particularly ADD and an erosive change in condyle; consistent with treatment- and disease-related vulnerability of joint tissues. Providing TMJ assessment into HNC imaging, coupled with early jaw-mobilization and masticatory-muscle–sparing strategies, may help preserve function.

Yalçın, Suna Uysal; Terzi, Rabia; Akkoç, Mehtap
Objective: This study was conducted to examine the relationship between eco anxiety and psychological well-being and emotion regulation skills in health professionals. Material and Methods: The data of the study, which was conducted in a descriptive-relational research design, were collected with an electronic questionnaire (Google Forms) in July-August 2023. The data were collected using a demographic data form created by reviewing the literature (open- and closed-ended questions) and questions related to climate and environment, Eco-Anxiety Scale (EAS), Psychological Well-Being (PWB) and Emotion Regulation Skills Scale (ERRS). Results: It was determined that 87.7% of the participants were female, 49.8% lived in metropolitan areas, 57.6% were nurses, and 94.4% did not receive climate change or environmental education. It was found that as the averages of “fear of other pandemics such as the coronavirus disease-2019 pandemic” and “fear of natural disasters such as earthquakes” increased, eco anxiety increased, and as the averages of “sensitivity to environmental issues” and “warning the environment about environmental pollution” increased, psychological well-being and emotion regulation skills increased. It was determined that the EAS-total scores of those living in the provincial centre and district were significantly higher than those living in the metropolitan area. When the relationship between EAS and PWB and ERRS was analysed, as the total scores of PWB and ERRS increased, the total scores of EAS decreased (p<0.05). Conclusion: In our study, we can say that eco anxiety has an effect on psychological wellbeing and emotion regulation. It is recommended to provide climate change trainings to health professionals and to take part in social responsibility projects to raise awareness and sensitivity on environmental issues.

Özsoy, Munteha; Tok, Yagmur Pirincci; Eskiler, Gamze Guney; Tok, Fatih; Karakus, Sevgi; Ozsoy, Yildiz; Özacar, Mahmut
Lenalidomide (L0) is an immunomodulatory agent with a range of effects, including anticancer and antiinflammatory activity, and is commonly utilized in treating multiple myeloma. A derivative of lenalidomide (L1) has been synthesized to enhance its effects and to target different cancer cell types. In this study, the lenalidomide derivative L1, with the chemical structure 1-[2-(2,6-dioxopiperidin-3-yl)-1-oxoisoindolin-4-yl]-3(p-tolyl)urea, was loaded onto a novel drug delivery system (DDS), and its activity was assessed towards triple-negative breast cancer cell lines (TNBC). MIL-100, a subclass of metal-organic framework (MOF) structures, was synthesized via a microwave-assisted hydrothermal method. MIL-100 was modified with quercetin (QC) as a linker, and its drug loading capacity was optimized, achieving a 95.18 % encapsulation efficiency. Additionally, the antioxidant properties of QC contributed to enhancing the performance of the DDS. In vitro drug release studies of the final product, MIL-100@QC@L1, were successfully conducted. The cytotoxic influences of the formulation on MDA-MB-231 cells were assessed using the WST-1 assay. After treatment with 10 mu g/mL of MIL100@QC@L1 for 24 h, the cell viability decreased significantly to 47.8 %, showing superior results compared to treatments with L0 and L1 alone.

- MakaleAçık Erişim2025Scopus SJRQ3Manufacture, characterization and in vitro drug release studies of chitosan-PVA-MNPs hydrogel essenced drug delivery system for anticancer drug quercetin
Özsoy, Munteha; Heidarnejad, Mahsa
In the current study, chitosan (CS) and polyvinyl alcohol (PVA) essenced hydrogels were produced using the freeze-thaw method without toxic cross-linking agents. Magnetic nanoparticles (MNPs) and quercetin (QC) were added to the system after synthesizing the hydrogel and the samples were freeze-dried using a lyophilizer. The prepared samples were used in in vitro drug release studies. QC, known as a natural polyphenol, is a promising candidate to support cancer treatment with its antioxidant effects. However, the hydrogels containing Fe3O4 nanoparticles exhibit high porosity and encapsulation efficiency, making them a convenient carrier for drug loading and controlled release. The QC was encapsulated in the synthesized CS-PVA-MNPs. Morphological changes of the prepared hydrogels were visualized using scanning electron microscopy (SEM). The molecular structure of the synthesized samples was determined using fourier transform infrared spectroscopy (FTIR), while their thermal stability was evaluated through thermogravimetric analysis (TGA). The encapsulation efficiency (EE) and drug loading efficiency (DLE) of QC in hydrogels including Fe3O4 MNPs were determined as 93.40% and 65.58%, respectively. In vitro release profiles of QC at pH 5 and pH 7.4 demonstrated the effectiveness of the hydrogel. These results indicate that CS-PVA-MNPs-QC is a convenient carrier for the intended drug delivery and reveal the potential of QC as a drug versus cancer cells.

- MakaleAçık Erişim2025Scopus SJRQ2Evaluation of the effect of subclinical hypothyroidism on retinal blood flow. OCT angiography study
Gunes, Irfan Botan; Onal, Eda Demir
Purpose: To evaluate retinal blood flow and retinal nerve fiber layer (RNFL) thickness in untreated subclinical hypothyroid patients by optical coherence tomography angiography (OCT-A). Methods: Forty-four eyes of 44 patients in the subclinical hypothyroidism group (SCHG) and 42 eyes of 42 healthy subjects in the control group (CG) were included in the study. Macular superficial retinal blood flow (SRBF), peripapillary RNFL thickness and optic nerve head cup to disk ratio (C/D) were measured and recorded by OCT angiography and compared between the patient and control groups. Correlation analysis between disease duration and SRBF and RNFL thickness was also performed. Results: Mean SRBF was 37.23 ± 2.11 in the SCH group and 38.45±1.20 in the control group. Mean SRBF was significantly lower in the SCH group (p=0.003). Mean Peripapillary RNFL thickness was 98.18±7.77 in the SCH group and 100.07 ± 7.71 in control group. Peripapillary RNFL thickness was not significantly different between groups (p=0.290). There was no significant difference in mean C/D between the groups (p=0.708). Correlation analysis showed a moderate negative correlation between duration of disease diagnosis and SRBF in the SCH group (p<0.001, r=-0.564). There was also a weak to moderate negative correlation between disease duration and RNFL thickness in SCH group (p=0.002, r=-0.489). Conclusions: A reduction in SRBF was observed in patients with subclinical hypothyroidism. Additionally, a negative correlation was identified between disease duration and RNFL thickness. Patients with subclinical hypothyroidism should be monitored for potential retinal vascular disease and the development of normotensive glaucoma.

The relatively complex functional anatomy of the mediobasal temporal region makes surgical approaches to this area challenging. Several studies describe various surgical approaches, along with their combinations and modifications, to reach lesions of this region. Some of these surgical approaches have been compared using artificial intelligence-based approaches that can be predicted, classified, and analyzed for complex data. Several surgical approaches, such as anterior transsylvian, trans-superior temporal sulcus, trans-middle temporal gyrus, subtemporal–transparahippocampal, presigmoid-retrolabyrinthine, supratentorial-infraoccipital, and paramedian supracerebellar-transtentorial, were selected for comparison. Magnetic resonance images (MRIs) were taken according to the criteria specified by the Radiology Department. With an open-source software tool, volumetric data from cranial MRIs were segmented and anatomical structures in the main regions were reconstructed. The Q-learning algorithm was used to find pathways similar to these standard surgical pathways. The Q-learning scores among the selected pathways are as follows: anterior transsylvian (Q_A) = 31.01, trans-superior temporal sulcus (Q_B) = 25.00, trans-middle temporal gyrus (Q_C) = 28.92, subtemporal-transparahippocampal (Q_D) = 23.51, presigmoid- retrolabyrinthine (Q_E) = 27.54, supratentorial-infraoccipital (Q_F) = 27.2, and paramedian supracerebellar-transtentorial (Q _G) = 21.04. The Q-value score for the supracerebellar transtentorial approach was the highest among the examined approaches and therefore optimal. A difference was also found between the total risk score of all points with pathways drawn by clinicians and the total risk scores of the pathways formed and followed by Q-learning. Artificial intelligence-based approaches may significantly contribute to the success of the surgical approaches examined. Furthermore, artificial intelligence can contribute to clinical outcomes in both preoperative surgical planning and intraoperative technical equipment-assisted neurosurgery. However, further studies with more detailed data are needed for more sensitive results.

Kılıç, Buket; Tekin, Ayla; Bünül, Sena Destan; Efendi, Hüsnü; Çakır, Özgür; Kılıç, Kamil Can; Çolak, Tuncay
Background: Multiple Sclerosis (MS), a neurodegenerative disorder affecting central nervous system, predominantly manifests as Relapsing–Remitting MS (RRMS), with subcortical volume loss serving as a prognostic indicator. Aims: This study aimed to assess time-dependent volumetric changes in thalamus and basal nuclei using a validated automated tool, volBrain software, on MR images (MRI) across three years and relationship between structural changes and disease progression. Methods: The study included 50 RRMS patients (33.5 ± 6.3 years; 68% female, 32% male) and 50 healthy controls (38.0 ± 5.8 years; 64% female, 36% male). T1-weighted brain MRI scans (from 2017, 2019, 2022) were analyzed via volBrain. Statistical analyses included Shapiro–Wilk, Mann–Whitney U, Friedman, and Dunn tests. Results: Thalamus, nucleus caudatus (nuc. caudatus), and nucleus lentiformis (nuc. lentiformis) volumes were significantly lower in RRMS (p < 0.05). Significant volume loss was observed between 1st-3rd MRI and 2nd-3rd MRI (p < 0.05), but difference between 1st-2nd MRI was not statistically significant (p > 0.05). The most pronounced volume loss occurred between 1st-3rd MRI, with the greatest reduction observed in left nuc. caudatus (7.92%). This loss was followed by right thalamus (5.68%) and nuc. lentiformis (4.35%). Although atrophy was observed across disease duration groups, this difference was not statistically significant (p > 0.05). Volume loss in thalamus and basal nuclei indicated significant atrophy in RRMS patients. Conclusions: Findings highlighted MS-induced subcortical atrophy that significantly impacts brain structure. This study was distinguished through volBrain, highlighting its effectiveness in providing data with clinical relevance. Subcortical volume changes show potential as biomarkers for disease progression and guiding targeted therapies.

- MakaleAçık Erişim2025Scopus SJRQ2Effects of pulse glucocorticoid treatment on retinal blood flow in patients with active Graves’ orbitopathy
Mohammadi, Hedayatullah; Güneş, İrfan Botan; Arı, Şeyhmus
Background: Various studies have shown decreased retinal blood flow in patients with active Graves’ orbitopathy (GO). We investigated whether retinal blood flow returned to normal levels after intravenous glucocorticoid treatment in active GO patients. Methods: Thirty-two eyes of 32 patients with moderate or severe active GO and 32 eyes of 32 healthy participants were included. The blood flow in the superficial plexus (SP), deep retinal plexus (DP), external retinal (ER) capillary plexus and choriocapillaris was measured via OCT-Angiography before treatment with methylprednisolone and one week after the completion of treatment. In addition, central macular thickness, intraocular pressure, and best corrected visual acuity, were compared with those of the control group. Results: Retinal blood flow (RBF) in the macula at SP, DP, ER and choriocapillaris levels was 48.38 ± 2.04, 54.17 ± 3.11, 0.42 ± 0.14 and 2176.45 ± 147.53 in the control group and 45.91 ± 4.56, 51.55 ± 3.84, 0.35 ± 0.34 and 2030.15 ± 132.15 in the active GO group, respectively. The RBF was significantly lower in the active GO group than in the control group at all angiographic levels (p < 0.001). Although blood flow increased after treatment, it remained lower than that in the control group (p < 0.001). The mean CMT was significantly greater in active GO patients before treatment than in the controls and after treatment (p < 0.001). No serious systemic or ocular side effects were observed in any of the patients. Conclusions: After pulse glucocorticoid treatment, retinal blood flow increases but remains low compared with that in the healthy control group.

- MakaleAçık Erişim2025Scopus SJRQ1Psychometric Properties of the Turkish Version of the Learning Flow in Clinical Simulation Scale
Yüksel, Azzet; Demi˙rel, Nehir; Çulha, Yeliz; Büyükyilmaz, Funda
Background Students must be able to accept the unrealistic aspects of clinical simulations to ensure these simulations are effective learning tools. As the flow during simulation must be measured to determine simulation learning effectiveness, a reliable measurement tool is needed. Purpose This study was carried out to translate the Learning Flow in Clinical Simulation Scale (LFCSS) into Turkish and then to validate it on a sample of nurses in Turkey. Methods A methodological research approach was used, and 135 nursing students with prior experience engaging with clinical simulations were recruited and enrolled as participants. The data were collected using two forms, i.e., the Student Information Form, LFCSS (Turkish version) and The Simulation Effectiveness Tool - Modified. Content validity was evaluated using a content validity index (CVI) based on expert opinions. The correlation between the two forms was calculated using the equivalent (parallel) forms method, and reliability and internal consistency were examined using the Cronbach alpha coefficient and item analysis. The construct validity of the scale was determined using factor analysis with varimax rotation. Results The Cronbach alpha coefficient was.93 for the entire LFCSS, with item-total score correlations ranging between.922 and.745. In terms of equivalent (parallel) forms reliability, a low correlation (r =.389) was found with the Flow Scale in Clinical Simulation and Prebriefing subdimension of the Simulation Effectiveness Tool - Modified, and a moderate correlation (r =.467, r =.584, r =.447) was found with the Learning, Confidence, and Debriefing subdimensions of the same. The factor analysis showed a four-factor structure explaining 74.13% of the total variance, which is similar to the original scale. The Kaiser-Meyer-Olkin value was.890 and the Bartlett's test chi-square value was 1455.35 with a p <.001 significance level. Conclusions The findings indicate the Turkish version of the LFCSS is a reliable and valid tool for assessing level of student/participant flow in clinical simulation.
