Acar, Esra; Colak, Tuncay; Eraldemir, Fatma Ceyla; Yener, Mehmet Deniz

Objective: Chronic stress is recognized as a factor that affects almost all organs and tissues and disrupts homeostasis. Stress also directly impacts the inflammatory process. In addition, klotho is a protein associated with lifespan. This study investigates the relationship that chronic stress has with klotho and tumor necrosis factor alpha (TNF-α), which is a key protein in the proinflammatory process. Materials and Methods: The study involves 16 Wistar albino rats divided into control and stress groups. The study applies the protocol for the chronic unpredictable mild stress (CUMS) model and uses the enzymelinked immunosorbent assay (ELISA) method to measure the TNF-α and Klotho levels in the serum, heart, aorta, and liver tissues. Results: The serum samples obtained from the stress group were found to have a significant decrease in klotho levels (p=0.013) and high TNF-α levels (p=0.042). However, no significant difference was observed in the TNF-α or klotho levels in the heart, aorta, and liver tissues between groups. Conclusion: Under chronic stress, a significant increase is observed in TNF- α, one of the main cytokines of inflammation, as well as a decrease in klotho hormone. This research endeavors to contribute to the existing body of knowledge concerning the interplay among stress, inflammation, and longevity.

Cases of Multiple Tracheobronchial Diverticulosis Characterized by Recurrent Pneumonia Episodes

Baris, Serap Argun; Basyigit, Ilknur; Boyacı, Haşim; Kaya, Hüseyin

Tracheal diverticulosis is a rare tracheal pathology that occurs in approximately 1-2% of the population. In order to draw attention to this rare tracheal pathology, we would like to present two cases in which tracheal diverticula were accompanied by bronchial diverticula. Case 1: A 58-year-old male patient presented to our outpatient clinic with a weight loss of approximately 7 kg in 1 month. Computed tomography (CT) revealed multiple tracheal diverticula in the trachea and bilateral main bronchi. Case 2: A 52-year-old male patient was admitted with complaints of right-sided pain and fever. CT and fiberoptic bronchoscopy revealed multiple diverticula in the bilateral bronchial system. Tracheal diverticula are paratracheal air cysts protruding from the tracheal wall. They are usually detected incidentally on imaging because patients are usually asymptomatic. Approximately 97% of the lesions are located in the right posterolateral wall and are usually solitary. The association between multiple and bronchial diverticula is very remarkable.

Acar, Esra; Dillioglugil, Meltem Ozlen; Sarihan, Mehmet; Yilmaz, Hasan; Yuksekkaya, Mustafa; Ates, Fatma; Ercan, Alev Meltem; Dillioglugil, Ozdal

Backround. The aim of this study was to examine the expression levels of the Thioredoxin interacting protein (TXNIP), Vitamin D receptor ( VDR), Human 8-oxoguanine DNA N-glycosylase 1 (hOGG1) genes in bladder cancer patients, according to clinical staging and determine the levels of potential therapeutic targets in serum samples. Material and Methods. Tissue and serum samples of patients who underwent transurethral resection (TUR) between 2017 and 2018 were obtained. Levels of TXNIP, hOGG1, and VDR genes were assessed using Real time-polymerase chain reaction (RT-PCR), while levels of Thioredoxin (Trx), 8-hydroxy-2'-deoxyguanosine (8-OHdG), and 1,25-dihydroxyvitamin D (25(OH)D3) were evaluated using the enzyme-linked immunosorbant assay (ELISA) method. Selenium levels were also measured using Optical Emission Spectroscopy (ICP-OES) in both tissue and serum samples. The protein-protein interactions and molecular and biological function of the proteins were assessed using Search Tool for the Retrieval of Interacting Genes/Proteins. Statistical analysis was conducted using IBM SPSS Statistics version 20.0. Results. The TXNIP gene showed higher expression in low-grade bladder cancer patients up to stage T1, but decreased in high-grade T1 and T2 stages. Both VDR and hOGG1 gene expressions were consistently lower across all clinical subgroups. No significant differences were found in serum 25(OH)D3, 8-OHdG, Hypoxia Inducible Factor 1 Alpha (HIF-1 alpha), selenium (Se), and tissue Se levels. Conclusions. TXNIP mRNA expression was remarkably lower in advanced stages. VDR and hOGG1 expression were low in all bladder cancer subgroups. These parameters could serve as potential targets for preventing or treating bladder cancer.

High Performance Liquid Chromatography-Tandem Mass Spectrometric determination of carcinogen nitrosamine impurities from pharmaceuticals and DNA binding confirmation aided by molecular docking application

Danie, Ibrahim; Agar, Soykan; Yurtsever, Mine; Uenal, Durieehvar oezer

Background and Aims: Nitrosamines are amine compounds attached to a nitroso group. The reaction of amines and nitrosating agents forms nitrosamines. Nitrosamine impurities are classified as Class 1 based on the carcinogenicity and mutagenicity data by ICH M7 (R1). The recent discovery of nitrosamines in some pharmaceutical products has caused concern. Nitrosamines are carcinogenic, so it is necessary to determine the possible nitrosamines in pharmaceutical products. Methods: An Inertsil ODS-3, (4.6x250 mm, 5 mu m) column was used for separation. A triple quadrupole mass detector with electrospray Ionisation (ESI) was used for detection. Multiple reaction monitoring (MRM) was used for quantification. The transition ions are 75.1 > 43.3 for NDMA and 103.0 > 75.1 for NDEA. The calibration curves consist of 5 concentration levels, including NDMA and NDEA (5, 10, 50, 100, 150 ng/mL). The mean r2 value was 0.997 for NDMA and 0.999 for NDEA. For NDMA and NDEA, LOD: 2 ng/mL, LOQ: 5 ng/mL. Results: Comprehensive in silico and in vitro results indicate that the method has good accuracy and precision. Conclusion: DNA binding interactions of the molecules NDMA and NDEA, were investigated through the molecular docking and molecular dynamics methods. Molecular Docking simulations showed that these small organic molecules have high-affinity scores and strongly bind to the minor groove of the hDNA via strong hydrogen bonds.

Volumetric and Asymmetric Index Analysis of Subcortical Structures in Multiple Sclerosis Patients: A Retrospective Study Using volBrain Software

Tekin, Ayla; Rende, Buket; Efendi, Husnu; Bunul, Sena Destan; Çakir, Ozgur; Çolak, Tuncay; Balci, Sibel

Introduction Multiple sclerosis (MS) is a chronic and autoimmune disease that has a significant influence on the central nervous system, such as the brain and spinal cord, affecting millions of individuals globally. Understanding the connection between subcortical brain regions and MS is crucial for effective diagnostic and therapeutic approaches for treating this disabling disease. This study explores the relationship between volume and contours of asymmetry index of subcortical brain regions in individuals with MS using volBrain software (https://www.volbrain.net; developed by Jose V. Manjon (Valencia Polytechnic University, Valencia, Spain) and Pierrick Coupe (University of Bordeaux, Bordeaux, France)). Methods In our retrospective investigation, we admitted 100 Turkish individuals, comprising 50 patients diagnosed with relapsing -remitting MS (RRMS) (24 (48%) males and 26 (52%) females) and 50 healthy controls (23 (46%) males and 27 (54%) females), registered between October 2017 and February 2022 for five years and underwent assessment in the radiology department at the Teaching and Research Hospital of Kocaeli University; 1,150 Turkish patients were excluded from our study based on our exclusion criteria. We used magnetic resonance imaging with a 3-Tesla (3T) scanner and volBrain software to assess volumes (cm(3)) and asymmetry indexes due to asymmetry for different levels of atrophy of total intracranial, total brain, gray matter, white matter, and subcortical regions, the most affected regions in MS patients for both patient and control cohorts. Results Statistical analysis revealed a significant difference between patient and control groups (p < 0.001), with patient group mean age at 38.32 years and control group mean age at 32.88 years. Patient group exhibited lower values for total intracranial, total brain, gray matter, white matter, and cerebrospinal fluid volume compared to control group (p < 0.05). The results indicated a statistically significant decrease (p < 0.05) in the values for total intracranial and total brain volume, whereas all other values remained unchanged. We compared volumes of subcortical structures on the right and left sides and found that the putamen, thalamus, and globus pallidus had statistically lower values in the patient group than in the control group (p < 0.001), apart from the lateral ventricle. Furthermore, our retrospective investigation demonstrated a statistically significant difference in the globus pallidus asymmetry index, indicating a preference for the patient group (p < 0.05). A lower asymmetry index value signifies a larger volume for the right side of the subcortical regions of the brain when compared to the left side. Conclusion Brain atrophy, although characterized by irreversible tissue damage, is targeted by therapeutic interventions to prevent progression. It is, therefore, imperative to develop a universally accepted measurement standard for subcortical structures that also considers the inherent variability present within each structure. Our findings serve as an important basis and indicator for the determination of subcortical atrophy and asymmetry in MS, the prognosis of the disease, and the etiology of clinical symptoms. Subsequent research may benefit by adopting the novel approach of considering brain atrophy as an outcome rather than a predictor, thereby facilitating the elucidation of the intricate biological mechanisms that give rise to volume loss.

Association Between Eyelid Twitching and Digital Screen Time, Uncorrected Refractive Error, Intraocular Pressure, and Blood Electrolyte Imbalances

    Gunes, Irfan B.

Introduction: Previous studies have shown that isolated eyelid myokymia (EM) is usually caused by stress, fatigue, and caffeine consumption. The purpose of this study was to evaluate the association between EM and digital screen time, uncorrected refractive error, intraocular pressure (IOP), and blood electrolyte levels. Methods: Between February 2023 and June 2024, 103 eyes of 103 patients who applied to the ophthalmology outpatient clinic with complaints of eyelid twitching lasting for more than two weeks and 103 eyes of 103 healthy individuals as a control group were included in the study. All participants were asked to record their daily time spent with digital screens for two weeks. Cycloplegic refractive error, IOP, optic nerve head cup/disc (C/D) ratio, and blood calcium, sodium, potassium, and magnesium levels were recorded and compared between the two groups. Results: Mean digital screen time was 4.84 +/- 1.74 hours in the control group and 6.88 +/- 2.01 hours in the EM group. It was found that digital screen time was significantly higher in the EM group compared to the control group (p<0.001). There was a strong positive correlation between the duration of eyelid twitching and the time spent in front of digital screens (p<0.001, r=0.670). There was no significant difference in cycloplegic refractive error, IOP, C/D ratio, and blood electrolyte levels between the two groups (p>0.05). Conclusion: Prolonged digital screen time might play a role in the development of EM. On the other hand, no relationship was found between eyelid twitching and uncorrected refractive error, glaucoma, or blood electrolyte levels.

The prognostic importance of the Naples prognostic score for in-hospital mortality in patients with ST-segment elevation myocardial infarction

Saygi, Mehmet; Tanalp, Ali Cevat; Tezen, Ozan; Pay, Levent; Dogan, Remziye; Uzman, Osman; Karabay, Can Yucel; Tanboga, Ibrahim Halil; Kacar, Flora Ozkalayci; Karagoz, Ali

BACKGROUND: The Naples prognostic score (NPS) is an effective inflammatory and nutritional scoring system widely applied as a prognostic factor in various cancers. However, the prognostic significance of NPS is unknown in ST-segment elevation myocardial infarction (STEMI). We aimed to analyze the prognostic value of the NPS in-hospital mortality in patients with STEMI. METHODS: The study consisted of 3828 patients diagnosed with STEMI who underwent primer percutaneous coronary intervention. As the primary outcome, in-hospital mortality was defined as all-cause deaths during hospitalization. The included patients were categorized into three groups based on NPS (group 1:NPS = 0,1,2; group 2:NPS = 3; group 3:NPS = 4). RESULTS: Increased NPS was associated with higher in-hospital mortality rates( P  < 0.001). In the multivariable logistic regression analysis, the relationship between NPS and in-hospital mortality continued after adjustment for age, male sex, diabetes, hypertension, Killip score, SBP, heart rate, left ventricular ejection fraction, myocardial infarction type and postprocedural no-reflow. A strong positive association was found between in-hospital mortality and NPS by multivariable logistic regression analysis [NPS 0-1-2 as a reference, OR = 1.73 (95% CI, 1.04-2.90) for NPS 3, OR = 2.83 (95% CI, 1.76-4.54) for NPS 4]. CONCLUSION: The present study demonstrates that the NPS could independently predict in-hospital mortality in STEMI. Prospective studies will be necessary to confirm the performance, clinical applicability and practicality of the NPS for in-hospital mortality in STEMI.

The evaluation of retrobulbar fat tissue in Graves’ orbitopathy with shear-wave ultrasound elastography

Gunes, Irfan Botan; Yilmaz, Hakan; Onal, Eda Demir

Purpose: To evaluate retrobulbar adipose tissue of patients with active and inactive Graves’ orbitopathy (GO) by shear-wave ultrasound elastography (SWE). Methods: Followed-up in our ophthalmology clinic due to GO, 72 eyes of 36 patients and 38 eyes of 19 healthy controls were included in this cross-sectional case–control study. Graves’ patients were divided into two subgroups under clinical activity score (CAS): active Graves’ orbitopathy (AGO) (CAS ≥ 3) and inactive Graves’ orbitopathy (IGO) (CAS < 3). SWE measurement values of retrobulbar adipose tissue of all participants were recorded in meters/second, and the intergroup comparisons were performed. Results: Thirty-four eyes of 17 patients in AGO, 38 eyes of 19 patients in IGO, and 38 eyes of 19 participants in the control group were included in the study. Mean values measured from retrobulbar adipose tissue through SWE were 1.00 ± 0.01 m/sec in AGO, 1.16 ± 0.01 m/sec in IGO, and 0.94 ± 0.01 m/sec in the control groups. Even so, the mean SWE value was significantly higher in the IGO group than in the other groups (p < 0.001). Mean SWE values were significantly higher in the AGO group than in the controls (p = 0.008). In the correlation analysis performed, a significant positive correlation was found between SWE and Hertel exophthalmometer measurement values (p = 0.026, r = 0.212), and thyroid-stimulating hormone receptor antibody (TSHR-Ab) levels (p = 0.018, r = 0.224). Conclusion: We detected SWE values of retrobulbar adipose tissue high in GO, especially in the IGO group. Such a situation, which we associated with the development of fibrosis, may be an indicator of unresponsiveness to immunomodulatory treatments.

Monkeypox: A Comprehensive Review of Virology, Epidemiology, Transmission, Diagnosis, Prevention, Treatment, and Artificial Intelligence Applications

Rahmani, Erfan; Bayat, Ziba; Farrokhi, Mehrdad; Karimian, Shiva; Zahedpasha, Reza; Sabzehie, Hamed; Poor, Sepehr Ramezani; Khouzani, Parisa Jafari; Aminpour, Solmaz; Karami, Mohammad

Monkeypox (Mpox), an uncommon zoonotic Orthopoxvirus, is commonly manifested by blisters on the skin and has a mortality rate of approximately 0-10%. Approximately two decades after the cessation of global smallpox vaccination, the number of confirmed cases of Mpox has been growing, making it the most common Orthopoxvirus infection. Therefore, in this narrative review, we aimed to shed light on recent advancements in the pathophysiology, transmission routes, epidemiology, manifestations, diagnosis, prevention, and treatment of Mpox, as well as the application of artificial intelligence (AI) methods for predicting this disease. The clinical manifestations of Mpox, including the onset of symptoms and dermatologic characteristics, are similar to those of the infamous smallpox, but Mpox is clinically milder. Notably, a key difference between smallpox and Mpox is the high prevalence of lymphadenopathy. Human-to-human, animal-to-human, and animal-to-animal transmission are the three main pathways of Mpox spread that must be considered for effective prevention, particularly during outbreaks. PCR testing, as the preferred method for diagnosing Mpox infection, can enhance early detection of new cases and thereby improve infection control measures. JYNNEOS and ACAM2000 are among the vaccines most commonly recommended for the prevention of Mpox. Brincidofovir, Cidofovir, and Tecovirimat are the primary treatments for Mpox cases. Similar to other viral infections, the best approach to managing Mpox is prevention. This can, in part, be achieved through measures such as reducing contact with individuals displaying symptoms, maintaining personal safety, and adhering to practices commonly used to prevent sexually transmitted infections.

Objectives: When considered within the scope of measures and measures taken under pandemic conditions, women’s mental health and violence against women, which is a global public health problem, are affected in adapting to changing living conditions. Our study aims to determine the intimate partner violence (IPV) and mental status of women during the pandemic period and to compare the mental health of women and the situation of IPV. Methods: This study is descriptive relation seeking and was carried out between March and April 2021 through online using Google Forms. During the pandemic period, 204 women were reached to determine their anxiety, depression, and IPV status. The data were collected with a form that included the demographic characteristics of women, their exposure to IPV and the types of violence they were exposed to, the “Generalized Anxiety Disorder-7 (GAD-7),” and the Patient Health Questionnaire (PHQ-9). Results: It was determined that there was an increase in women’s exposure to IPV during the pandemic period, emotional violence, and controlling behavior were the highest, and normal-mild anxiety and mild-moderate depression were the majority in the GAD-7 and PHQ-9 scale sub-dimensions. In addition, it was found that anxiety and depression scores increased in parallel with the increase in physical, sexual, and emotional violence and controlling behaviors. Conclusion: As a result of this study, the problems experienced during the pandemic period caused an increase in women’s exposure to partner violence and anxiety and depression levels. It is noteworthy that IPV has an effect on the increase in anxiety and depression levels. In this context, our study will be a solid foundation for developing an effective psychological intervention for women during the pandemic period.

The Inhibition of RXRα and RXRβ Receptors Provides Valuable Insights for Potential Prostate Cancer Treatment, in silico Molecular Docking and Molecular Dynamics Studies

Agar, Soykan; Akkurt, Barbaros; Ulukaya, Engin

Introduction: Prostate cancer has emerged as a widespread health concern, with systemic inflammation believed to substantially contribute to its development and progression. The presence of systemic inflammatory responses has been established as an independent predictor of unfavorable long-term outcomes in prostate cancer patients. The goal of this study is to inhibit RXRα and RXRβ receptors, which are involved in prostate cancer, with Luteolin, Formononetin, and Kaempferol, with varying success. Methods: Retinoid X receptors (RXRs) hold crucial roles within the nuclear receptor (NR) superfamily, and compelling evidence from preclinical studies underscores the therapeutic potential of targeting RXRs for treating neurodegenerative and inflammatory conditions. Consequently, the ability to regulate and modulate RXRs using phytoestrogen ligands, Formononetin, Kaempferol, and Luteolin, assume paramount importance in treatment strategies. Results: The comprehensive in silico findings of this study vividly demonstrate the remarkable efficacy of Luteolin in inhibiting and modulating RXRα and RXRβ, while Formononetin emerges as a notably potent suppressor of RXRβ. Kaempferol, as the third compound, also exhibits commendable inhibitory attributes, although its impact is slightly less pronounced compared to the other two. Discussion: These findings highlight the notable binding and inhibition capabilities to RXRα and RXRβ, offering valuable insights for potential prostate cancer treatment avenues warranting further exploration through in vitro and in vivo analyses.

Hanci, Deniz; Onaran, Öykü İzel; Altun, Huseyin; Uyar, Yavuz

Background: Nasal septal perforation (NSP) is a structural flaw that affects the mucosa, cartilage, and bone of the nasal septum, often stemming from septoplasty as the most frequent cause. The natural healing of a perforated septum is uncommon; on the contrary, it tends to deteriorate progressively. Various surgical methods have been outlined for the repair of NSP. In this research, the authors introduce an innovative approach utilizing fascia lata graft with PRP for the restoration of NSP across diverse sizes. Methods: The researchers conducted a retrospective analysis involving 25 patients who underwent repair for nasal septal perforation (NSP) from January 2021 to January 2023. Grafts were obtained, and the perforation was addressed using an open rhinoplasty technique, followed by the insertion and suturing of the graft. Results: The mean size of the septal perforations was 17 mm horizontally and 18 mm vertically. The mean follow-up period was 12 months. Complete closure of NSP was achieved in 23 out of 25 patients (92%). Among the cases, 17 were male (68%), and the age ranged from 25 to 45 years with a mean of 38.7. Eight cases (32%) were smokers. At 12 months postoperatively, three large-sized NSPs were closed successfully, while two medium-sized NSPs did not achieve closure due to smoking. Conclusion: The Fascia Lata and Platelet-Rich plasma fusion technique for the closure of nasal septal perforations is a secure and dependable method, demonstrating a notable success rate. This approach should be regarded as a viable option for individuals dealing with nasal septal perforation. Fascia Lata-Fat Island Graft in Septal Perforation Repair

Hancı, Deniz; Çelik, Cem; Altun, Hüseyin; Uyar, Yavuz

Objective: To measure the success of the fascia lata-fat island graft technique in septal perforation repair as measured by nasal endoscopic examination. Background: This study presents the results of using fascia lata-fat island, a different graft technique, for the repair of septal perforations, offering an alternative to this challenging procedure. Methods: This retrospective study assesses nasal septal perforation repair using the fascia lata-fat island graft technique performed by a single surgeon. Inclusion criteria involved completing 12-month follow-ups within a 3-year review period. Success rates were calculated and evaluated alongside patient characteristics. Results: The median (range) age of the 25 patients included in the study was 34 (25-45) years and 72.0% were men. The septal perforation size of all patients was >2 cm and the etiological cause in all of them was previous septal surgery. All patients were followed for 12 months. The perforation was completely closed in 23 of 25 patients (92%). Conclusion: Using a different graft with an open rhinoplasty approach, we achieved a high success rate in patients with large septal perforations, followed for 1 month with nasal stenting and an average follow-up duration of 12 months. Psychometric Properties of the Spiritual Care Needs Scale for the 9–18 Age Group in Turkey

Özdemir, Emine Zahide; Özdemir, Tuğba; Bektaş, Murat

This methodological study aimed to establish the validity and reliability of the Spiritual Care Needs Scale in the 9–18 age group within the Turkish context. Expert opinions were considered in the analysis of the study, which was completed with the participation of 840 children. The content validity index and Cronbach’s alpha coefficients were examined, along with the results of the Shapiro–Wilk normality test and the explanatory and confirmatory factor analyses. The scale had 21 items under two subdimensions: “meaning and hope” and “caring and respect.” According to the EFA, the scale explained 57.5% of the total variance. The meaning and hope subdimension explained 53.3% of the total variance, whereas the care and respect subdimension explained 4.16%. The total Cronbach’s alpha value of the scale was 0.96, and the fit indexes were as follows: X2 = 513.807, df = 184, X2/df = 2.792, RMSEA = 0.065, GFI = 0.89, IFI = 0.94, NFI = 0.91, TLI = 0.93, CFI = 0.94, AGFI = 0.87, and GFI = 0.89. Therefore, the Spiritual Care Needs Scale is a valid and reliable measurement tool for the 9–18 age group in the Turkish population.

BrainPixGAN: Generating intraoperative MRI images with mask-based generative networks

Eker, Ayşe Gül; Pehlivanoğlu, Meltem Kurt; Duru, Nevcihan; Dündar, Tolga Turan

In recent years, efforts to enhance precision in brain tumor surgeries have focused on using artificial intelligence (AI) with medical imaging. This involves integrating AI with medical imaging. This study aimed to generate a tumor-free MRI by using Generative Adversarial Networks (GANs) to establish a relationship between preoperative magnetic resonance imaging (MRI) and resection cavity segmentation masks obtained from intraoperative ultrasound (IOUS) data. For cavity segmentation, U-Net and U-Net with transfer learning were used, with the U-Net + EfficientNetB7 model achieving a high dice score of 97.82. The resection cavity mask was applied to preoperative MRI images using Pix2Pix, SPADE GAN, and BrainPixGAN. BrainPixGAN, incorporating transfer learning, outperformed the others, achieving SSIM 0.87, PSNR 35.89, and LPIPS 0.0037. This innovative approach represents a pioneering effort in generating GAN models for intraoperative MRI (iMRI) images using IOUS data, despite the challenges in setup and cost associated with iMRI imaging.
