- MakaleAçık Erişim2025Scopus SJRQ2Optimizing Miniscrew Stability: A Finite Element Study of Titanium Screw Insertion Angles
Akbulut, Yasin; Ozdemir, Serhat
This study aimed to evaluate how different insertion angles of titanium orthodontic miniscrews (30°, 45°, and 90°) influence stress distribution and displacement in surrounding alveolar bone using three-dimensional finite element analysis (FEA), with a focus on biomechanical outcomes at the titanium–bone interface. The 90° insertion angle generated the highest stress in cortical bone (58.2 MPa) but the lowest displacement (0.023 mm), while the 30° angle produced lower stress (36.4 MPa) but greater displacement (0.052 mm). The 45° angle represented a compromise, combining moderate stress (42.7 MPa) and displacement (0.035 mm). This simulation-based study was conducted between January and April 2025 at the Department of Orthodontics, Kocaeli Health and Technology University. A standardized 3D mandibular bone model (2 mm cortical and 13 mm cancellous layers) was constructed, and Ti-6Al-4V miniscrews (1.6 mm × 8 mm) were virtually inserted at 30°, 45°, and 90°. A horizontal orthodontic load of 2 N was applied, and von Mises stress and displacement values were calculated in ANSYS Workbench. Stress patterns were visualized using color-coded maps. The 90° insertion angle generated the highest von Mises stress in cortical bone (50.6 MPa), with a total maximum stress of 58.2 MPa, followed by 45° (42.7 MPa) and 30° (36.4 MPa) insertions (p < 0.001). Stress was predominantly concentrated at the cortical entry point, especially in the 90° model. In terms of displacement, the 90° group exhibited the lowest mean displacement (0.023 ± 0.002 mm), followed by 45° (0.035 ± 0.003 mm) and 30° (0.052 ± 0.004 mm), with statistically significant differences among all groups (p < 0.001). The 45° angle showed a balanced biomechanical profile, combining moderate stress and displacement values, as confirmed by post hoc analysis. From a biomimetics perspective, understanding how insertion angle affects bone response provides insights for designing bio-inspired anchorage systems. By simulating natural stress dissipation, this study demonstrates that insertion angle strongly modulates miniscrew performance. Vertical placement (90°) ensures rigidity but concentrates cortical stress, whereas oblique placement, particularly at 45°, offers a balanced compromise with adequate stability and reduced stress. These results emphasize that beyond material properties, surgical parameters such as insertion angle are critical for clinical success.

- MakaleAçık Erişim2025Scopus SJRQ1Exploring the potential of interdental brush in oral cytology: a pilot study on sampling efficiency
Çelebi, Elif; Öçbe, Melisa; Sinanoğlu, Enver Alper; Tekkeşin, Merva Soluk
Background: With the renewed interest in brush biopsies, particularly for screening oral potentially malignant disorders and cancers, understanding the efficacy of available brush types in oral cytology is crucial. This study aims to compare the effectiveness of interdental brush sampling with commonly used cytology brushes in oral cytology evaluation. Methods: The study design was an observational clinical study. Brush biopsies were collected from the buccal mucosa of 99 healthy volunteers using an Interdental Brush- TePe Original Interdental Brush (TePe, Malmö, Sweden) (Cell sampling-Interdental Brush (CSIB)), Cytobrush Plus GT (Medscand Medical AB, Sweden) (Cell sampling device 1 (CSD1)), and CervexBrush (Rovers, Oss, Netherlands) (Cell sampling device 2 (CSD2)). Samples were evaluated for cellularity, depth of the epithelial layer, and cellular integrity. Results were compared between brush types using the Friedman test, and multiple comparisons were examined using the Dunn test. The significance level was set at p < 0.05. Results: Of the 99 cases, 59.6% were female and 40.4% were male, with a prevalence of smoking and alcohol consumption at 43.4%. The mean age was 24.3 years (± 4.5). There were no significant differences in cellularity between the brush types (p = 0.205). Mild cellularity was observed at rates of 3%, 9.1%, and 9.1% for CSD1, CSIB, and CSD2, respectively. Parabasal/basal layer cells were detected in 1% of samples collected with CSD1 and 4% with CSD2, while CSIB samples contained only superficial/intermediate cells. CSIB produced an inadequate integrity rate of 30.3%, whereas neither CSD2 nor CSD1 yielded samples with inadequate integrity. Conclusions: Interdental brushes may have potential as accessible and practical tools for chairside cytological screening. The CSIB obtained high-cellularity samples; however, it demonstrated low scores in cellular integrity. Future oral cytological studies may explore the modifications in interdental brush design to improve its potential in sampling for cell preservation and integrity.

Abakay, Hanife; Yetkin, M. Fatih; Ayşe, G.Ü.Ç.; Şekercan, Gizem
This study aimed to investigate the relationship between dysphagia severity and neck proprioception, pain, and mood in individuals with multiple sclerosis (MS). The individuals were divided into two groups according to the severity of dysphagia: dysphagia group (n = 13) with a score of 3 and above on the Dysphagia Assessment Scale in Multiple Sclerosis (DYMUS) scale and mild dysphagia group (n = 13) with a score of 2 and below. Clinical swallowing function was evaluated with the Turkish version of the Eating Assessment Tool-10 (T-EAT-10) and DYMUS. Proprioception was assessed with a laser by noting the deviation from the center target in centimeters. Neck pain severity was assessed with the Visual Analogue Scale (VAS), and mood was assessed with the Beck Depression Inventory (BDI). Individuals with severe dysphagia demonstrated statistically significantly greater proprioceptive impairment in cervical extension and right rotation. A significant difference was also found between the two groups in terms of clinical swallowing evaluations, neck pain status, and mood measurements. The findings suggest that proprioceptive deficits, increased pain, and mood disturbances are closely associated with dysphagia severity in MS. A multidimensional approach addressing dysphagia, proprioception, and pain management may be beneficial in optimizing dysphagia rehabilitation in patients with MS.

Hyperspectral imaging (HSI) is an imaging method that differs from traditional methods by recording the light reflection and absorption properties of each point across a wide color range. HSI enables the non-invasive determination of chemical and structural differences in tissues with high accuracy. In the domain of medicine, hyperspectral imaging represents a novel methodology that facilitates novel approaches in brain surgery, including the distinction of tumors and blood vessels, cancer diagnosis, and the monitoring of tissue oxygen levels and blood flow during surgical procedures. Additionally, hyperspectral imaging has a wide range of applications in various fields, including agriculture, food, environment, geology, and defense. This review comprehensively examines current datasets, application areas, and computational approaches related to the use of hyperspectral imaging technology in brain surgery. HSI enables the precise differentiation of different brain tissues such as tumors, vessels, functional areas, and the spine by collecting data across a wide spectral band range for each pixel. Recent studies in the literature demonstrate that machine learning and deep learning-based methods can achieve high accuracy rates in tissue segmentation and the detection of pathological structures in brain surgery. Additionally, datasets developed at different centers and involving various patient groups provide an important foundation for enhancing the clinical efficacy of HSI.While HSI boasts significant advantages in terms of its non-invasiveness and real-time analysis potential, which benefit surgeons during intraoperative decision-making processes, challenges such as data size, computational costs, and device integration pose barriers to its routine use. In the future, it is anticipated that these limitations will be overcome through the development of efficient algorithms, AI-supported systems, and multi-center validation studies. In conclusion, HSI technology exhibits considerable potential to become a standard intraoperative instrument in neurosurgery, driven by advancing computational methods and increasing clinical validation.

- BildiriYalnız künye2025Calculating Brain Shift in Brain Tumor Surgeries: Integration of MR and Ultrasound Imaging Methods and Comparison of Feature Extraction Algorithms
Eker, Ayse Gul; Kurt Pehlivanoglu, Meltem; Duru, Nevcihan; Dundar, Tolga Turan; Serdar Mutluer, Ahmet; Adali, E.
Brain tumors are abnormal tissue masses formed as a result of uncontrolled cell proliferation in the brain, and surgery is the most common treatment method. During surgery, changes in the position of brain tissue are referred to as 'brain shift,' which prevents preoperative MR images from accurately reflecting the intraoperative situation and reduces surgical success. This study addresses the integration of MR and ultrasound (US) imaging methods to calculate brain shift and improve surgical accuracy in brain tumor surgeries. Different feature extraction algorithms such as SIFT, ORB, AKAZE, and BRISK were used to solve the alignment problem between MR and US images. Using an adaptive search strategy and 3D coordinate transformation, the amount of brain shift was calculated and real-time information was provided to surgeons. The use of the adaptive search strategy dynamically adjusted the search area according to the degree of brain shift, eliminating unnecessary regions and significantly increasing processing speed. Experimental results showed that the ORB algorithm performed best for small and moderate shifts (e.g., 3.78 ± 1.65 mm error for small shifts), while the SIFT algorithm was more successful for large shifts (9.91 ± 2.30 mm error). This method aims to enable surgeons to make more accurate decisions during surgery and to increase patient safety. In the future, it is planned to improve the accuracy of the method by using deep learning-based approaches and larger datasets.

Sonuç: Madde kullanımı ile ilgili akademik çalışmalarda Türkçe-İngilizce anahtar kelimelerin uyumu; kullanıcı ve bağımlı terimlerinin güncel tanı ölçütlerine uygun biçimde kullanımı; madde isimlerinin güncel ve amacına uygun kullanımı ve özel olarak kannabinoidler ile ilgili terimlerin uygun kullanımı açılarından terminolojik sorunlar dikkat çekmektedir. Madde isimlerinin uygun kullanılmaması sorunu sağlık bilimleri dışındaki disiplinlerde daha yaygındı. Bağımlılık alanında akademik dilin uygun kullanımına yönelik çalışmalar, bu alandaki disiplinler arası iş birliğini güçlendirebilir.
- MakaleAçık Erişim2024Assessment of the impact of anemia on the treatment of acute bronchiolitis cases
Dönmez, Tuğrul; Dundaroz, Mehmet Rusen
Aim: Acute bronchiolitis is a clinical condition characterized by inflammation of the bronchioles, primarily caused by viral agents in children under two years of age. The cornerstone of treatment includes hydration and oxygenation, while patients should be closely monitored for potential complications. Our objective is to evaluate the impact of anemia on treatment outcomes in children diagnosed with acute bronchiolitis. Materials and Methods: Children aged 1-24 months diagnosed with mild or moderate acute bronchiolitis in a tertiary pediatric health and disease clinic were retrospectively included in the study. Demographic characteristics and bronchiolitis severity (mild/moderate) were recorded. Pre- and post-treatment clinical severity, respiratory rate, heart rate, oxygen saturation levels, and bronchiolitis clinical scores were analyzed. The clinical findings and treatment responses of anemic and non-anemic patients were compared. A p-value of <0.05 was considered statistically significant in all analyses. Results: A total of 85 patients with a median age of 5.4 months (52.9% male) were included in the study. Anemia was present in 25.9% of the patients. At admission, the respiratory rate (RR) was significantly lower in anemic patients compared to nonanemic patients (median 37 vs 41, p=0.003). No significant differences were observed between anemic and non-anemic groups for clinical respiratory scores (CRS) (median 4.7 vs 4.7, p=0.721), heart rate (HR) (median 135.6 vs 134.3, p=0.798), or oxygen saturation (SaO2) (median 93.1 vs 93.0, p=0.304) at admission. Post-treatment, both groups demonstrated significant improvements in CRS, HR, and SaO2 (p<0.05 for all). However, RR remained lower in the anemic group compared to the non-anemic group (median 32.1 vs 36, p=0.003). The overall change in CRS and other parameters between the anemic and non-anemic groups was not statistically significant (p>0.05). Conclusion: Mild anemia does not affect the response to treatment for hypoxia in children with mild to moderate acute bronchiolitis.
