- MakaleAçık Erişim2024Scopus SJRQ3Inhibition of pancreatic cancer via LPAR4 receptor with a de novo drug complex design using theoretical organic chemistry: Comprehensive molecular docking, molecular dynamics
Agar, Soykan; Arasan, Yaren; Akkurt, Barbaros; Ulukaya, Engin
The present work relates to a de novo organic chemistry involved drug design and repurposing discovery of a Quercetin and Ascorbic Acid complex formation with the IUPAC nomenclature of ‘’3-((2S)-2-(3,4-dihydroxy-5-oxo-2,5-dihydrofuran-2-yl)-2-hydroxyethoxy)-2-(3,4-dihydroxyphenyl)-3,5,7-trihydroxychroman-4-one’’ to suppress pancreatic cancer via the inhibition of LPAR4 receptor. This was achieved with molecular docking and molecular dynamics studies and found that Ascorbic Acid is docking manoeuvre assistant for Quercetin to form Hydrogen bonds and Covalent bonds to shut down LPAR4 receptor with excellent inhibition constant. This study may very well lead to further in vitro organic synthesis, characterization and cell line results and in vivo/ex ovo animal testing for etherical bound Quercetin and Ascorbic Acid complex.

Background: The mesenchymal stem cells (MSCs) with characterized by their multipotency and capacity to differentiate into various tissue cell types, have led to their incorporation in regenerative medicine research. However, the limited numbers of MSCs in the human body and their diverse differentiation capabilities in tissues highlight the need for exploring alternative regenerative cell sources. In this study, therefore, we conducted molecular level examinations to determine whether pericytes, specialized cell communities situated near blood vessels, could serve as a substitute for human bone marrow-derived mesenchymal stem cells (hBM-MSCs). In this context, the potential application of pericytes surrounds the vessels when MSCs are insufficient for functional purposes. Methods: The pericytes utilized in this investigation were derived from the placenta and characterized at the third passage. Similarly, the hBM-MSCs were also characterized at the third passage. The pluripotent properties of the two cell types were assessed at the gene expression level. Thereafter, both pericytes and hBM-MSCs were directed towards adipogenic, osteogenic and chondrogenic differentiation. The cells in both groups were examined on days 7, 14, and, 21 and their differentiation status was compared both immunohistochemically and through gene expression analysis. Results: Upon comparing the pluripotency characteristics of placental pericytes and hBM-MSCs, it was discovered that there was a substantial upregulation of the pluripotency genes FoxD3, Sox2, ZPF42, UTF1, and, Lin28 in both cell types. However, no significant expression of the genes Msx1, Nr6a1, Pdx1, and, GATA6 was observed in either cell type. It was also noted that pericytes differentiate into adipogenic, osteogenic and, chondrogenic lineages similar to hBM-MSCs. Discussion: As a result, it has been determined that pericytes exhibit high differentiation and proliferation properties similar to those of MSCs, and therefore can be considered a suitable alternative cell source for regenerative medicine and tissue engineering research, in cases where MSCs are not available or insufficient. It is notable that pericytes have been suggested as a potential substitute in studies where MSCs are lacking.

Kalabalık, Fahrettin; Yılmaz, Neslihan; Aydın, Elif Gül; Aytuğar, Emre
Background/purpose: The objective of this study was to assess the root apical closure ages of first permanent molars retrospectively among a group of western Turkish children aged between 5 and 15 years using cone-beam computed tomography (CBCT). Materials and methods: CBCT images of 202 patients were examined. The patients were divided into 7 groups according to age: ≤8 years, 8–8.99 years, 9–9.99 years, 10–10.99 years, 11–11.99 years, 12–12.99 years, and ≥13 years. Teeth with at least one open apex were considered as ‘teeth with open apex’ and teeth with all closed apices were considered as ‘teeth with closed apex’. The data was evaluated by Pearson Chi-square test and Fisher's exact test. P value of <0.05 was considered statistically significant. Results: Apical closure rate of the maxillary first molars was 4.1%, 32.2%, 55.4%, 76.2%, and 90.6% at 8–8.99 years, 9–9.99 years, 10–10.99 years, 11–11.99 years, and 12–12.99 years, respectively. Apical closure rate of the mandibular first molars was 4.4%, 41.2%, 70.7%, 80.0%, and 92.2% at 8–8.99 years, 9–9.99 years, 10–10.99 years, 11–11.99 years, and 12–12.99 years, respectively. At 12–12.99 years, females had a significantly higher rate of closed root apices of mandibular first molars than males (P < 0.05). At 9–9.99 and 12–12.99 years, females had a significantly higher rate of closed root apices of maxillary molars than males (P < 0.05). Conclusion: CBCT is a useful tool for the evaluation of root development. The results of present study can provide valuable aids for clinicians during root canal treatment of permanent first molars.

Type 1 diabetes-mellitus (T1DM) is characterized by damage of beta cells in pancreatic islets. Cell-sheet engineering, one of the newest therapeutic approaches, has also been used to create functional islet systems by creating islet/beta cell-sheets and transferring these systems to areas that require minimally invasive intervention, such as extrahepatic areas. Since islets, beta cells, and pancreas transplants are allogeneic, immune problems such as tissue rejection occur after treatment, and patients become insulin dependent again. In this study, we aimed to design the most suitable cell-sheet treatment method and macrocapsule-device that could provide long-term normoglycemia in rats. Firstly, mesenchymal stem cells (MSCs) and beta cells were co-cultured in a temperature-responsive culture dish to obtain a cell-sheet and then the cell-sheets macroencapsulated using different concentrations of alginate. The mechanical properties and pore sizes of the macrocapsule-device were characterized. The viability and activity of cell-sheets in the macrocapsule were evaluated in vitro and in vivo. Fasting blood glucose levels, body weight, and serum insulin & C-peptide levels were evaluated after transplantation in diabetic-rats. After the transplantation, the blood glucose level at 225 mg dl-1 on the 10th day dropped to 168 mg dl-1 on the 15th day, and remained at the normoglycemic level for 210 days. In this study, an alginate macrocapsule-device was successfully developed to protect cell-sheets from immune attacks after transplantation. The results of our study provide the basis for future animal and human studies in which this method can be used to provide long-term cellular therapy in T1DM patients.

Objective: The aim of this study was to compare the migration index (MI), femoral inclination angle (FIA), lateral center-edge angle (LCEA), and acetabular index (AI) values measured from hip joint radiographs of children with bilateral spastic cerebral palsy (CP) divided into ambulatory and nonambulatory groups according to the Gross Motor Functional Classification System with functional levels of children, and to examine their relationship with anthropometric measurements of the lower extremities. Materials and Methods: The present investigation encompassed the evaluation of 30 children afflicted with CP, through the conduction of hip joint radiographic assessments, which involved the right and left hip joints, the acquisition of images using MI, FIA, LCEA, and AI parameters, as well as measurement of lower-extremity anthropometric data. Results: A significant difference was found between ambulatory and nonambulatory groups in the left MI, FIA, and LCEA (P < 0.05). In comparison of anthropometric measurements to height ratios, no significant difference was found, except for left ankle circumference and bi-iliac diameter measurements (P > 0.05). Conclusion: The hip joint angular values of the ambulatory group were found to be within normal limits compared to those of the nonambulatory group. Although there were significant differences between hip joint angles in the two groups, there was no significant correlation with lower-extremity anthropometric measurements. It was concluded that hip joint angles were more pathological and prone to hip displacement in the nonambulatory group. This study revealed the importance of strategies to maintain and improve the functional capacity of children with CP.

Yılmaz, Hakan; Güneş, İrfan Botan
Objective: To compare main lacrimal gland values through shear-wave elastography (SWE) in patients with low Schirmer value and unspecified Sjögren's syndrome (SS) with healthy controls. Material and method: Admitted to the ophthalmology department with Schirmer value <10 mm, randomly selected 46 eyes of 46 patients evaluated for Sjögren's syndrome (SS) in the rheumatology department between December 2022 and April 2023 were classified as low Schirmer group (LSG). Randomly selected 48 eyes of 48 patients at a similar age with Schirmer value >10 mm were included as controls. Main lacrimal gland SWE measurements in LSG and control groups were recorded and compared as meter/second (m/sec). Results: Mean SWE values of the main lacrimal gland were measured as 2.78±0.66 m/sec and 2.26±0.29 m/sec in LSG and controls. SWE measurements were significantly higher in LSG patients than in controls (p<0.001). No significant correlation was found in the analysis between the Schirmer and the main lacrimal gland SWE values in LSG patients (p=0.702, r=0.058). No significant correlation was also detected between the Schirmer and main lacrimal gland SWE values in controls (p=0.097, r=0.242). No significant relationship was also found between age, gender, body mass index (BMI), and SWE values (p=0.351, p=0.493, p=0.328, respectively). Conclusion: Mean SWE value of the main lacrimal gland was determined as significantly higher in patients with aqueous lacrimal insufficiency without SS than in controls. We consider that SWE measurements may be an imaging method to support the diagnosis of aqueous lacrimal insufficiency and used in follow-ups of those with dry eye syndrome (DES) in the future.

Kurt Pehlivanoğlu, Meltem; Ay, Eren Cem; Eker, Ayşe Gül; Albayrak, Nur Banu; Duru, Nevcihan; Mutluer, Ahmet Serdar; Dündar, Tolga Turan; Doğan, İhsan
Background: Despite using a variety of path-finding algorithms that use tracts, the most significant advancement in this study is considering the values of all brain areas by doing atlas-based segmentation for a more precise search. Our motivation comes from the literature’s shortcomings in designing and implementing path-planning methods. Since planning paths with curvatures is a complex problem that requires considering many surgical and physiological constraints, most path-planning strategies focus on straight paths. There is also a lack of studies that focus on the complete structure of the brain with the tracks, veins, and segmented areas. Instrument dependence is another inadequacy of the methods proposed in the literature. Aims: The aim of this study is to design a new surgical path planning framework that helps to plan the surgical path independently of the instrument, considers the entire structure of the brain, and allows curvilinear surgical paths. Thus, neurosurgeons can generate patient-specific possible optimal surgical pathways before the neurosurgical procedure. Materials & Methods: The proposed framework includes different path-finding algorithms (Dijkstra, A*, and their aggressive variants) that find optimal paths by taking the risk scores (surgeons assessed all the segmented regions, considering the extent of damage. In this evaluation, scores ranged from “0 to 10,” with the most critical areas receiving a score of “10,” while the least possible affected areas were assigned a score of "0") for sensitive brain areas into consideration. For the tract image processing the framework includes fractional anisotropy (FA), relative anisotropy (RA), spherical measure (SM), and linear measure (LM) methods. Results: This is the first paper to handle tracts and atlas-based segmentation of the human brain altogether under a framework for surgical path planning. The framework has a dynamic structure that gives the flexibility to add different path-finding algorithms and generate different widths of surgical pathways. Moreover, surgeons can update the score table to guarantee minimally invasive surgery. The output file format of all the extracted surgical paths is NRRD, so it can be easily visualised, analysed, or processed over the third part software tools. Discussion: In this study, we generated many possible surgical pathways then these pathways were evaluated by the surgeons the results were impressive because the framework could identify surgical pathways used in real-world surgery that correspond to the standard pathways such as anterior transsylvian, trans sulcal, transgyral, and sub-temporal. Conclusion: This study proposes a new surgical path planning framework for neurosurgery. Moreover, in the future by adding/adopting different parameters (such as operation time, and short and long-term complications after surgery) to the proposed framework, it would be possible to find new surgical pathways for difficult surgical conditions.

This chapter provides a detailed explanation of the clinical investigations, pharmacological effects, and characteristics of quinone and quinone derivatives, along with references to pertinent sources. Although quinones have an oxygen group in their structure, they are quickly oxidized and interact with reactive oxygen species. This demonstrates how quinones work as drugs. Quinones and their derivatives are used as medications to treat a wide range of diseases, including cancer, lung, kidney, brain, and tumor disorders. Each of the following has effects on a different area of health; lapochols, anthraquinones, naphthoquinone and its derivatives, and other wide range of quinone derivatives are thoroughly explored in this chapter.
- MakaleYayıncıda açık erişim2024Scopus SJRQ3The Tricellufuse Technique for Closing Nasal Septal Perforations
Hanci, Deniz; Onaran, Öykü İzel; Altun, Huseyin; Uyar, Yavuz
Objective: Nasal septal perforation (NSP) is a structural anomaly that affects the mucosa, cartilage, and bone of the nasal septum, often stemming from septoplasty as the primary cause. Spontaneous healing of a perforated septum is rare, and typically the healing process is observed until it is determined that the condition is stable and no longer deteriorating, at which point surgical intervention is considered. Numerous surgical techniques have been proposed for NSP repair. This study introduces a novel approach incorporating a fascia lata graft with fat tissue and diced septal cartilage to reconstruct medium-sized NSP. Methods: The researchers conducted a retrospective analysis involving 22 patients who underwent repair for NSP from January 2021 to January 2023. Grafts were obtained, and the perforation was addressed using an open rhinoplasty technique, followed by the insertion and suturing of the graft. Results: The mean size of the septal perforations was 12 mm horizontally and 13 mm vertically. All patients with NSPs were chosen as medium size (within 0.5-2 cm). The mean follow-up period was 12 months. Complete closure of NSP was achieved in 19 out of 22 patients (86.3%). Among the cases, 15 were male (68%), and the ages ranged from 25 to 45 years with a mean of 35. Eight cases (36%) were smokers. After 12 months postoperatively, closure was not achieved in 3 medium-sized NSPs due to smoking. The analysis revealed that the failure rate in the smoker group was statistically significant (P =.036). Conclusion: The TriCelluFuse technique, combining fascia lata, fat tissue, and diced septal cartilage, is a secure and reliable method for closing NSPs. It has a significant success rate and should be considered a viable option for individuals with NSPs.

- MakaleYalnız künye2024Scopus SJRQ2Views of nurses and midwives working in primary healthcare services on intimate partner violence against women
Uysal Yalçin, Suna; Aksoy, Sena Dilek; Zonp, Zeynep; Bilgin, Hülya
Background Violence against women is a global public health problem. Primary healthcare services and health workers play an important role in managing this problem. The aim was to identify barriers and facilitators to the identification and management of intimate partner violence (IPV) among women presenting to primary healthcare services from the perspective of nurses/midwives, and to identify the challenges and needs to improve practices. Methods This was a qualitative descriptive study, in which semistructured interviews were conducted with 19 nurses/midwives working in primary healthcare services. The interviews were analysed using a thematic analysis approach through an inductive process. The Consolidated Criteria for Reporting Qualitative Research was used to ensure comprehensive reporting of the protocol for this qualitative study. Results Four main themes were identified: (1) detection of intimate partner violence, (2) nurses'/midwives' current practices, (3) barriers to identification and management, and (4) needs. Conclusions Primary care nurses/midwives in this study perceived that they were not responsible for the detection and management of intimate partner violence, and a majority reported they did not want to take an active role. However, those who were willing to engage in the detection and management of IPV required sufficient time for patient consultations, a reduced workload and a separate room with suitable physical conditions. It is recommended that nurses/midwives be provided with training on IPV detection and management at specific intervals, and studies be conducted to evaluate the outcomes of such training, and initiate the professional decision-making process of nurses/midwives to effectively identify, respond to and support individuals affected by IPV.

Hanci, Deniz; Gurpinar, Berk; Altun, Huseyin; Onaran, Oyku Izel; Uygan, Ugur; Kumas, Omer; Uyar, Yavuz
Objective: Nasal osteotomy is a crucial step in rhinoplasty, explicitly addressing the open roof deformity in nasal bones. This study aimed to assess and compare the recovery patterns associated with 4 distinct osteotomy techniques in a rabbit model. Materials and Methods: Twenty white New Zealand rabbits weighing 2.5 to 3.0 kg were included in the study. Median osteotomies were performed on the left side using piezosurgery and on the right side using an oscillating micro-saw. Additionally, internal osteotomy was executed on the right side and external osteotomy on the left side, both performed with a chisel across all rabbits. Ten rabbits were sacrificed on the 7th day, while the remaining 10 were sacrificed on the 15th day for subsequent histological examination. The granulation tissue formation (GTF), vascularization, inflammation, collagen fibril content, osteotomy gap width, and new bone formation were evaluated in hematoxylin and eosin and Masson’s trichrome-stained sections. Results: On the 7th day, piezosurgery demonstrated statistically-significant advantages in terms of GTF, neovascularization, and collagen fibril quantity when compared to the oscillating micro-saw (P <.05). Furthermore, a statistically-significant difference was observed among the groups concerning the distribution of connective tissue organization between the 7th and 15th days (P <.05). Conclusions: This study’s findings unequivocally establish that bone healing after osteotomy with piezosurgery surpasses that achieved with an oscillating micro-saw in this rabbit model.

- Kitap BölümüYalnız künye2024Commercialization of quinone-based drugs
Altuner, Elif Esra; Sen, Fatih; Dar, Umar Ali
In this chapter, the antibiotic drug effects of quinone and quinone derivatives are discussed in detail. Today, antibiotic drugs used against harmful pathogens work for a certain period, but these drugs become useless after a certain time or after a certain dose. This is because harmful microorganisms become resistant to these antibiotics after a certain threshold. For this reason, searches have been made that can replace antibiotics and are harmless. Quinone and quinone derivatives are one of these researches. Quinone and its derivatives show antibacterial, antiviral, and antifungal properties against various microorganisms. Quinones have the structure of cyclic diketones. Therefore, it is an advantage that it gives radicals easily as it resonates easily and thus has antioxidant properties in this reaction. The main quinone-based drugs used as antibiotic drugs are as follows: anthracyclines, mitomycin C, mitoxantrone, aziridinylbenzoquinones, streptonigrin, lapachols, and actinomycin D. Some of these drugs have anticancer and some have antitumor properties. This chapter can offer details on quinone and quinone derivatives-related commercial drug research.

Kopuz, Dilan; Sancak, Elifilgi; Tekçe, Neslihan
The purpose of this study was to examine the effect of different decontamination agents on the bond strength of saliva-contaminated CAD/CAM blocks and repair composite materials. Two hunderd eighty 3-mm-thick specimens were prepared from four different CAD/CAM materials: Katana Zirconia UTML disc, IPS e.max CAD block, Shofu block, and Vita Enamic block. Each material was divided into seven different subgroups (N=70). Group 1 had a clean surface. The other groups were comprised of the samples, which were contaminated with human saliva: group 2: negative control (non-cleaned); group 3: cleaned with water spray; group 4: cleaned with 70% ethanol; group 5: cleaned with Ivoclean; group 6: cleaned with Katana Cleaner; and group 7: cleaned with phosphoric acid. After the decontamination protocols, the resin composite cylinders were bonded to the CAD/CAM surfaces with a thin layer of dual-cured resin cement. The samples were stored for 24 hours at 37°C in distilled water. Then, they were subjected to a shear bond strength test (SBS). The values were recorded, and fracture types were evaluated using a microscope. Data were analyzed using two-way ANOVA and Tukey’s post-hoc test. Generally, all decontaminating agents improved the SBS of composites to Katana Zirconia UTML, IPS e.max, and Vita Enamic materials (p<0.05). However, for Shofu materials, group 6 samples exhibited significantly higher bond strength values as compared with group 2 samples (p=0.026). The highest SBS values were seen in the phosphoric acid-treated group in Katana zirconia materials (26.45 ± 9.38 MPa), whereas the lowest values were seen in group 2 samples in Shofu materials (13.17 ± 3.40 MPa). Each decontaminant agent improved the bond strength of composites to the contaminated CAD/CAM materials. If saliva is not cleaned before adhesive procedure, SBS values may decrease. All decontamination agents can be used safely on zirconia, lithium-disilicate glass-ceramic, hybrid ceramic, and polymer-infiltrated ceramic surfaces.
