Machine Learning Approaches to Predict Thyroid Cancer Recurrence: A Comparative Study

Ozturk, Candide; Sagir, Ozgur; Vural, Ulas; Adali, E.

Thyroid cancer recurrences have significant impacts on patients' health. The current clinical methods have limited prediction accuracy because of inadequate incorporation of individual patient variability. Therefore, there is a need for advanced techniques that can help medical experts to make accurate and reliable predictions. Recent efforts on the topic are focused on machine learning algorithms. This study aims to assess the performance of these algorithms for predicting thyroid cancer recurrence. Four prediction models are trained using Gradient Boosting, Random Forest, XGBoost, and AdaBoost algorithms. The performances of the models are evaluated with metrics such as accuracy, F1 score, F-05 score, and area under the ROC curve (AUC). Experiments with the K-Fold (5-fold) method have revealed how effective these models can be in the prediction of thyroid cancer recurrence. The performance of the machine learning models was evalu-ated using accuracy, Fl-score, F05-score, and AUC score metrics. The XGBoost model outperformed others, achieving an accuracy of 95,3%, Fl-score of 91,8%, F05-score of 91,9%, and AUC score of 98,2%. These results show that machine learning-based solutions may contribute to incorporating more accurate and individualized treatment decisions in clinical practice.

The association of pre-photorefractive keratectomy Schirmer-1 test value with postoperative corneal epithelial thickness, ocular surface discomfort, and visual acuity

Güneş, İrfan Botan; Öztürk, Hakan; Özen, Bediz

PURPOSE: To investigate the association of pre-photorefractive keratectomy Schirmer-1 test value with post-photorefractive keratectomy central corneal epithelial thickness, ocular surface disease index score, and uncorrected distance visual acuity. METHODS: Patients were categorized according to preoperative Schirmer-1 value: the normal Schirmer Group (n=54; Schirmer-1 test value, >10 mm) and the low Schirmer Group (n=52; Schirmer-1 test value, between 6 and 10 mm). We analyzed ablation depth, visual acuity, result of Schirmer-1 test (with anesthesia), tear film break-up time, ocular surface disease index score, central corneal epithelial thickness, and spherical equivalent refraction. RESULTS: We found significant differences between the groups in Schirmer-1 test value, tear film break-up time, and ocular surface disease index score, both preoperatively and postoperatively (p<0.001). The preoperative central corneal epithelial thicknesses of the two groups were similar (p>0.05). After photorefractive keratectomy, the Schirmer-1 test value and spherical equivalent refraction decreased in both groups (p<0.05), and ocular surface disease index scores and central corneal epithelial thickness values increased in the low Schirmer Group (p<0.001) but not in the normal Schirmer Group (p>0.05). The postoperative central corneal epithelial thicknesses of the low Schirmer Group were significantly higher than those of the normal Schirmer Group (p<0.001). Postoperative uncorrected distance visual acuity did not differ significantly between the two groups (p>0.05). CONCLUSIONS: In patients with low Schirmer-1 test values before photorefractive keratectomy, the corneal epithelium thickened and ocular surface complaints increased during the postoperative period. However, changes in the corneal epithelium did not affect the postoperative uncorrected distance visual acuity. To reduce postoperative problems on the ocular surface in these patients, we recommend that dry eye be treated before photorefractive keratectomy.

Kuculmez, Ozlem; Dündar Ahi, Emine; Cosar, Sacide Nur; Guzel, Sukran

Introduction: High-frequency laser therapy has been increasingly used in several musculoskeletal disorders, but there is still a lack of evidence for the usage of the device in neck pain. This study aimed to compare the effectiveness of physiotherapy, high-frequency laser, and exercise therapy methods in the treatment of pain in cervical disk herniation. Methods: It was a multicenter, randomized, controlled clinical trial. Patients aged between 18 and 65 with neck pain and a diagnosis of cervical disk hernia were included in the study. Patients with a history of cervical surgery, rheumatism, cancer, or pacemaker were excluded from the study. The patients were randomized into 15 sessions of physiotherapy, high-frequency laser, or exercise therapy groups and evaluated with a range of motion, visual analog score, Neck Disability Index, and Short Form Health Survey-36 before treatment, after treatment, and in 1st and 3rd months. p < 0.05 was considered statistically significant. Results: In total, 150 patients were analyzed. There was a significant improvement in range of motion, visual analog score, Neck Disability Index, and Short Form Health Survey-36 scores in three groups after 3-month follow-up (p < 0.05). The improvement was statistically greater in the physiotherapy and high-frequency laser therapy groups (p < 0.05), but there was no significant difference between these two groups (p > 0.05). Discussion: The results in the physiotherapy and high-frequency laser therapy groups were better than the exercise group. They may be alternatives to each other in cervical disk hernia treatment.

    Akpolat, Refiye; Arslan, Sevban

This study aimed to determine the effects of preoperative active and passive warming on postoperative hypothermia, vital signs, and perception of thermal comfort in patients scheduled to undergo elective open abdominal surgery. This was a randomized controlled study. The study sample comprised 90 patients (30 in the active warming group, 30 in the passive warming group, and 30 in the control group) who agreed to participate and met the research criteria. According to the comparison of patients’ vital signs, a statistically significant difference was observed in terms of patients’ preoperative body temperature values (v2 = 56.959; p = 0.000). A comparison of the patients’ postoperative thermal comfort perception scores yielded a statistically significant difference (v2 = 39.693; p = 0.000). Postoperative comfort scores in the active warming group were significantly higher than those in the passive warming and control groups. In conclusion, warming methods are effective in preventing undesirable postoperative hypothermia. The time to reach normothermia after surgery was shorter, vital signs were at a desirable level, and thermal comfort perceptions were higher in patients who were prewarmed. Shakila, Shakila; Kilic, Burcu; Bardakkaya, Merve; Aksakal, Fatma; Sagkan, Rahsan Ilikci; Dogruer, Deniz S.

In the current study, three series of new compounds were designed and synthesized through hit optimization, based on the hit compound with a structure (E)-N-(4-{[benzyl(methyl)amino]methyl}thiazol-2-yl)-3-(3,4-dimethoxyphenyl)prop‑2-enamide, which was found as a potential multifunctional anti-Alzheimer agent in our previous study. Then, the biological activities of synthesized compounds (4a-c, 13a-b and 15a-c) were evaluated. The results indicated that 4b, a 2-phenoxyacetamide derivative, exhibited good multifunctional activities. Namely, compound 4b displayed better BChE inhibition (BChE IC50 = 2.10 µM) and antioxidant activity (ORAC = 1.18 Trolox equivalents) compared to the hit compound (BChE IC50 = 14.70 µM, ORAC = 0.5 Trolox equivalents). Additionally, 4b was able to chelate all tested biometals and it also exhibited a neuroprotective effect (63 %), comparable to reference ferulic acid (77 %) while maintaining its safety upon PC12 cell line. When it comes to lowering the levels of TNF-α, NO, IL-1β, and IL-6, 4b performed better activity, relative to the other synthesized compounds. Subsequently, in silico studies such as physicochemical properties, density functional theory (DFT) computational approach and molecular docking studies were performed.

Dementia Classification from MR Images Using Machine Learning and Deep Learning Models; [Makine Öǧrenmesi ve Derin Öǧrenme Modelleri Kullanilarak MR Görüntülerinden Demans Siniflandirma]

Pehlivanoǧlu, Meltem Kurt; Varol, Onur; Aldemir, Osman; Uncu, Ata Emir; Duru, Nevcihan; Adali, E.

The classification of dementia levels in Alzheimer's disease is divided based on various characteristics of the disease. In this study, for the purpose of classifying dementia levels in Alzheimer's disease using MRI images, various Convolutional Neural Network (CNN)-based deep learning models, as well as machine learning and Artificial Neural Network (ANN) models, were trained. Various image manipulation and augmentation techniques were applied to create new datasets, and model performances were observed on these datasets. As a different approach, numerical features were extracted from MRI images, and models were built using these extracted features. Fusion hybrid models were produced by combining the most successful models trained with the images and their numerical representations. Experimental results have proven that hybrid models outperform other models in terms of performance in the classification of Alzheimer's disease.

Dear Editor; We read with interest the article titled “Use of methacoline challenge test to detect bronchial hyperresponsiveness in children with persistent rhinitis” published in Cukurova Medical Journal by Guc et al. [1]. This study showed that BHR positivity is high in children with allergic rhinitis. Allergic rhinitis can cause false positive BHR. In addition, smoking and COPD can cause false positives [2, 3]. COPD can occur in childhood [4]. We think that it would be appropriate to mention this in the discussion part of this study.

Oxidized Low Density Lipoprotein Receptor 1 3'UTR 188C>T Gene Polymorphism in Patients with Coronary Artery Bypass Grafting

Bahtiyar, Nurten; Baykara, Onur; Hacioglu, Yalcin; Öner, Tuba; Cinemre, Fatma Behice; Aydemir, Birsen; Onaran, Ilhan; Arslan, Caner; Tel, Cigdem; Hamid, Rauf

Aim: Coronary artery disease (CAD) is a pathological process characterized by atherosclerotic plaque accumulation in the epicardial arteries. Inflammation and high lipid levels play a role in pathological changes in atherosclerosis. Besides traditional risk factors, genetic factors such as single nucleotide polymorphism (SNPs) can be involved in disease process. In this study, we aimed to evaluate the effects of oxidized low density lipoprotein receptor 1 (OLR1) 3'UTR188C>T gene polymorphism, C -reactive protein (CRP), and lipid status in patients with coronary artery bypass grafting (CABG). Materials and Methods: The study population consisted of 109 CAD patients who had undergone CABG, and 127 healthy controls. The OLR1 3'UTR188C>T polymorphism was genotyped using PCR-RFLP technique. Serum CRP, high -density lipoprotein -cholesterol (HDL-C), and low -density lipoprotein -cholesterol (LDL-C) levels were measured with an automatic biochemistry analyzer. Results: The distribution of the OLR1 3'UTR188C>T genotypes and alleles did not differ significantly between CAD patients with CABG and controls. Serum CRP levels were increased in patients compared to the control group (p<0.001), but HDL-C, and LDL-C levels were not different between two groups. Traditional risk factor such as cigarette smoking, alcohol use, family history, diabetes mellitus and hypertension were increased in patients compared to the control group (p<0.001, for each). The CRP levels were higher in patients with the TT, CT, and CC genotypes than in controls with the same genotypes (p<0.001, p<0.01, and p<0.05, respectively). Conclusion: OLR1 3'UTR 188C>T polymorphism may not be involved in susceptibility to atherosclerosis. However, traditional risk factors in atherosclerosis such as smoking, alcohol consumption, family history, hypertension, diabetes mellitus, and circulating CRP levels were increased in our CABG population. The evaluation of OLR1 3'UTR188C>T and different OLR1 SNPs may be useful for their single and combined effects in atherosclerosis.

Investigation of Postoperative Risks of Nausea and Vomiting in Patients With Breast Cancer

Şişman, Hamide; Arslan, Sevban; Gezer, Derya; Akpolat, Refiye; Alptekin, Dudu; Gökçe, Esma

Purpose: To examine the risks of postoperative nausea and vomiting (PONV) in breast cancer patients. Design: Descriptive, cross-sectional study. Methods: The study was conducted in the surgical oncology department of a tertiary hospital between June 2020 and December 2020. A total of 83 female patients who were scheduled for surgical treatment for breast cancer were included in the study. The patients were evaluated using the Patient Evaluation Form created by the researcher by scanning the literature, the VAS Pain Rating Scale, and the Apfel Risk Scoring System. Findings: A significant relationship was found between age and PONV at the 2nd, 4th, 8th, 24th and 48th hours after surgery (respectively: P = .00, P = .00, P = .00, P = .00, P = .00). There was a significant correlation between the duration of surgery and PONV at 0 hour, PONV at first oral intake and PONV at 4 hours (respectively; P ˂ .01, P ˂ .01). The highest rate of PONV of all time (50.6%) at the 0th hour when the VAS scores of the patients was the highest. The PONV rate at the 48th hour was the lowest of all time periods (1.2%) (respectively; P ˂ .01, P ˂ .01). Conclusions: While women with breast cancer who have undergone mastectomy have the highest risk of postoperative and nausea vomiting in terms of age and pain severity, the duration of the operation and the first oral intake time also pose a risk. Nurses play a key role in the quality of care, patient safety, and patient satisfaction. It is recommended that institutions create evidence-based strategies and take necessary precautions in the preoperative evaluation of patients in terms of postoperative nausea and vomiting.

Comparison of the effectiveness of dry needling and high-intensity laser therapy in the treatment of myofascial pain syndrome: a randomized single-blind controlled study

    Ahi, Emine Dundar; Sirzai, Hulya

Myofascial pain syndrome (MPS) is a very common disease in the population that seriously affects quality of life. Although many treatment modalities are used, there is still no common protocol. The aim of this study was to compare the effectiveness of high-intensity laser therapy (HILT) and dry needling options. This prospective study included 108 patients with neck and/or upper back pain, diagnosed with MPS, who were randomly separated into 3 groups: the exercise group, the exercise + HILT group (HILT group), and the exercise + dry needling group (needling group). The visual analog scale (VAS), neck disability index (NDI), short form-36 (SF-36) scores, and neck range of motion (ROM) values of the patients before and after treatment were recorded and compared between the groups. In all 3 groups, the VAS and NDI scores decreased and ROM levels increased after treatment. The results in the HILT and needling groups were statistically significantly better than those of the exercise group (p < 0.05).The addition of HILT and dry needling to exercises is seen as a more successful treatment option to reduce pain in MPS. Clinical trial registration number: NCT05078333.

Synthesis and evaluation of multitarget new 2-aminothiazole derivatives as potential anti-Alzheimer's agents

Bardakkaya, Merve; Kilic, Burcu; Sagkan, Rahsan Ilıkcı; Aksakal, Fatma; Shakila, Shakila; Dogruer, Deniz S.

In this study, two diverse series of 2-aminothiazole-based multitarget compounds, one propenamide and the other propanamide derivatives, were designed and synthesized. Subsequently, their anticholinesterease and antioxidant (ORAC) activities were tested. Among them, compound 3e was the most potent acetylcholinesterase (AChE) inhibitor (AChE IC50 = 0.5 μM, butyrylcholinesterase [BChE] IC50 = 14.7 μM) and compound 9e was the most potent BChE inhibitor (AChE IC50 = 3.13 μM, BChE IC50 = 0.9 μM). Kinetic experiments showed that both compounds were mixed-type inhibitors. According to the anticholinesterease activity results, five compounds (3e, 4e, 5e, 9d, and 9e) were selected for further activity studies, all of which are dual cholinesterase inhibitors. Then, selected compounds were investigated in terms of their metal chelation activity. Moreover, their neuroprotective effects against H2O2-induced damage in the PC12 cell line were evaluated at 10 μM and the results showed that the neuroprotective effect of 3e was 53% compared with the reference ferulic acid (77%). 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT) results of selected compounds revealed that the compounds were noncytotoxic. Additionally, 3e was more effective in reducing lipopolysaccharides-induced interleukin-1β (IL-1β), IL-6, tumor necrosis factor-α (TNF-α), and nitric oxide (NO) production in the human monocyte derived from patient with acute monocytic leukemia cell line compared with other selected compounds. Finally, a molecular docking study was also performed.

Objectives: The aim of this study is to examine the relationship between occlusal factors and temporomandibular joint diseases (TMD). Material and Methods: In our study, 43 individuals who had fixed orthodontic treatment and 20 individuals who did not need treatment at Istanbul Aydin University Faculty of Dentistry Department of Orthodontics were included in the study. Evaluations were made at the beginning of treatment (T0) and at 6 months (T1). T-Scan® computerized Occlusion Analyzer was used to evaluate the number of contact points, occlusal force parameters during centric occlusion and lateral movements, and occlusion and disclusion times. Differences between groups were evaluated with Mann–Whitney or independent t-test, and within-group differences were evaluated with Wilcoxon sign test or paired t-test, depending on whether the data showed normal distribution or not. Results: There was no statistically significant distinction between the gender-based parameters and right and left joint of Joint Vibration Analysis record in all three groups (P > 0.05). At the beginning of orthodontic treatment, there was no statistical difference in the variables within the group and between the groups. As a result of the comparison between the beginning of treatment (T0) and the 6th month (T1), no significant difference was found between the parameters of the anterior/posterior occlusal force distribution of the right-left quadrant and the force distribution of the working and non-working sides in lateral movements, and the occlusion and disclusion time parameters. Maximum intercuspal position left/right (MxlntlTSCANT1) was found to be statistically significant at 6 months in individuals who received fixed orthodontic treatment with extraction compared to the none xtraction treatment group (P < 0.05). Conclusion: According to our research, T-scan is one of the best devices for the early detection of TMD, especially for people undergoing orthodontic treatment. T-scan offers fast, non-invasive, and repeatable occlusion recording.

The effects of Kinesio taping and breathing exercises on pain management after gynaecological abdominal surgery: A randomized controlled study

    Yılmaz, Sakine; Terzioğlu, Füsun

Aim: This study aimed to evaluate the effects of Kinesio taping and breathing exercises on pain management after gynaecological abdominal surgery. Methods: This randomized controlled study was conducted with 132 participants at a public hospital between June 2017 and 2018. The Kinesio taping method was applied once, immediately after the surgery during the hospital stay and breathing exercises were performed three times a day for 30 min on the first, second and third post-operative days. Results: The median age of the women was 47 years. The mean body mass index was 27.6 ± 5.0 kg/m2. When the intervention and control groups were compared, the level of women's pain in the Kinesio taping group was significantly reduced. There was also a statistically significant difference in terms of the passage of gas and defecation time and wound healing in groups with Kinesio taping. Conclusions: Kinesio taping has been shown to be an effective intervention in the management of acute pain after surgery. The results of this study suggest that future studies may be indicated to study taping in other surgical conditions.
